DescriptionThe Patient Transfer Center - Nurse Supervisor is responsible for overseeing the efficient and effective transfer of patients within the hospital system. This role ensures that patient flow is optimized resources are utilized effectively and high-quality care is maintained throughout the transfer process.
ResponsibilitiesSupervision & Leadership
- Supervise daily operations of the Transfer Center including staff scheduling performance management and workflow coordination.
- Provide clinical guidance and support to transfer center nurses and staff.
- Serve as a point of escalation for complex transfers or clinical decisions.
- Facilitate staff training orientation and competency validation.
Operational Management
- Ensure all patient transfer requests are triaged prioritized and processed in a timely and clinically appropriate manner.
- Collaborate with bed management emergency departments hospitalists and transport services to facilitate smooth transitions.
- Monitor and report key performance indicators (e.g. transfer times acceptance rates delays).
Communication & Collaboration
- Act as a liaison between referring and receiving facilities ensuring clear and accurate exchange of clinical information.
- Foster collaboration across departments including physicians case management and nursing leadership.
- Maintain positive relationships with internal and external stakeholders to support patient flow initiatives.
Quality & Compliance
- Ensure adherence to regulatory requirements and hospital protocols regarding transfers and admissions.
- Participate in audits incident reviews and quality improvement initiatives.
- Identify and resolve system barriers to timely patient transfers.
Qualifications- 5 years work experience in a clinical setting
- BSN
- Current State RN License
