The Department of Economic Security Division of Developmental Disabilities (DDD) is seeking an experienced and highly motivated individual to join our team as a Nursing Supervisor - Case Management. DDD provides individuals with developmental disabilities and their families services and supports that are flexible high quality and member driven. These services provide individuals with opportunities to exercise their rights and responsibilities of independent decision-making and engagement in the community.

Applicants for this position cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement and cannot be employed or subcontracted by a DDD Qualified Vendor.

Essential Duties and Responsibilities include but are not limited to:

Supervises and directs work activities of staff; provides necessary training; technical advice and assistance regarding development implementation or monitoring member Person Centered Service Plans (PCSP). Ensures consistency in case management and overall applications with an emphasis on programming and ensuring all federal and state requirements are complied with and documentation and files is complete.
Assigns caseloads monitors work activities nursing assessments conducts meetings and provides assistance in resolving conflicts. Responsible for recruitment assures appropriate staffing hiring retention and evaluate of work performance of staff. Reviews cases assigned to work unit; assesses needs and assigns cases to staff members; reviews PCSP and Nursing Assessments developed by staff to ensure member needs are met.
Ensures staff is performing appropriate comprehensive assessments of members physical mental & long-term needs to develop appropriate care plans for prevention outcome improvement decreased risk status and increased self-care opportunities. Provides assistance and support to members/families in crisis directly or through participation with the interdisciplinary team. Identifies and resolves problems timely and appropriately.
Attends meetings with providers managerial and administrative staff. Implements strategies goals objectives policies procedures and guidelines to define systems and area operations relevant to the Division. Works with programs/resource development to ensure services and providers are acquired to meet the needs of members. 10 Acts as liaison to behavioral health case management and the mental health agencies/providers to problem solve difficult cases. Attends staff meetings. Works health care issues with Health Care Services clinical team and inter-disciplinary meetings. Establishes and maintains good rapport and open communication through various forums inter-disciplinary meetings area meetings team meetings.
Researches and takes appropriate actions regarding grievances and appeals. Reviews verifies and authorizes expenditures to ensure compliance with budgetary constraints. Prepares reports relating to cases assigned to the work unit.

Knowledge in:
Federal state and local laws rules regulations policies and procedures relating to service delivery personnel contracting fiscal management and Title XIX.
Comprehensive knowledge of developmentally disabled nursing theory and practice.
Principles of human service planning and program development.
Nursing theories and practices related to individuals with developmental disabilities and behavioral health conditions.
Special needs of the developmentally disabled (DD) population.
Research and analysis components of organizational problem solving.

Skills in:
Planning developing implementing administering managing monitoring and evaluating developmental disabilities support services training programs and health related services.
Coordinating and administering through subordinate supervisors a network of home and community based services to members with developmental disabilities.
Analyzing complex problems and identifying and implementing solutions.
Decision making crisis management negotiation organization supervision of personnel delegation and leadership.
Primary skills acquired during formal nursing education.
Ability to:
Analyze complex situations accurately and quickly adopt an effective course of action.
Manage a complex array of projects with rapidly shifting priorities and timelines.
Establish and maintain effective working relationships/communication with diverse individuals and community groups such as members parents/families professionals providers advocate and community planning organizations elected officials agency employees media and the general public.
Provide leadership to groups and individuals.
Consistently exhibit above average skills in reading comprehension calculation negotiation delegation policy formulation and leadership.
Motivate lead and supervise subordinate staff.

Required at time of appointment: possession of a current license to practice as a registered nurse in the State of Arizona.
If this position requires driving or the use of a vehicle as an essential function of the job to conduct State business then the following requirements apply: Drivers License Requirements. Any employee operating a motor vehicle on State business must possess and retain a current valid class-appropriate drivers license complete all required training and successfully pass all necessary drivers license record checks. The license must be current unexpired and neither revoked nor suspended. Employees who drive on state business are subject to drivers license record checks must maintain acceptable driving records and must complete all required driver training. An employee who operates a personally owned vehicle on state business shall maintain the statutorily required liability insurance (see Arizona Administrative Code (A.A.C.) R.11).
Employee shall have a valid Level One fingerprint clearance card issued pursuant to Arizona Revised Statute 41-1758.07 in order to work with children and vulnerable adults.
Candidates for this position shall be subject to a search of both the Department of Child Safety Central Registry pursuant to A.R.S. 8-804 and the Adult Protective Services Registry pursuant to A.R.S. 46-459.
Successfully pass background and reference check; employment is contingent upon completion of the above mentioned process and the Agencys ability to reasonably accommodate any restrictions.
All newly hired State employees are subject to and must successfully complete the Electronic Employment Eligibility Verification Program (E-Verify).
