Job Description:

Job Duties:

This position supports coordination of the Heart Disease and Stroke Prevention and Management Nutrition and Physical Activity focused initiatives and Diabetes Prevention and Management Program throughout the state.

  • Provide Heart Disease Diabetes Obesity Nutrition and Physical Activity prevention outreach education and intervention.
  • Support the office through data management and reporting and be a community partner liaison between our federal funding sources and programs in the community
  • Support the outreach to new or repeat partners who have interest in implementing our programs
  • Facilitate and monitor partner touchpoints and support the outcomes of Chronic Disease Management.
  • Develop and manage grant and contract agreements; complete budget monitoring and planning;
  • Develop program policies procedures and evaluation; on-going program quality assurance and monitoring;
  • Provide outreach and technical support to various entities; ensures compliance with state and federal regulations;
  • Provide training; research and writes required plans grants and proposals;
  • Maintain internal databases and resources.



Requirements / Qualifications:

Minimum Qualifications: Bachelors degree in business or public administration social/behavioral sciences health sciences public health education law or related field OR experience in the development of operational policies and procedures performing quality assurance analysis/evaluation of programs; grant writing contract management or leading community based health/social care groups. Related coursework training and/or education may substitute for the Bachelors degree on a year-for year basis.

Preferred Qualifications:

Masters degree in public or community health healthcare nutrition or related field. Minimum of one year of experience working in public or community health healthcare or related field. Experience in community health education programs prevention of heart disease and stroke and Diabetes prevention and control; and connecting community programs with health systems to improve heart disease and stroke prevention care and management.

KNOWLEDGE SKILLS AND ABILITIES:

Knowledge of the principles and practices of public health planning and implementation; the principles of grant writing reporting and grant administration; the practices and techniques of developing partnerships and collaborations.

Ability to maintain accurate and thorough records design educational and informational materials training aids and presentations; prepare grant proposals; plan and lead group sessions; interpret and apply federal grant requirements agency rules policies and procedures; communicate effectively; prioritize and manage time; problem solve and make decisions; demonstrate professionalism; balance needs requirements and work load for multiple projects; self-motivate and work independently with minimal supervision.
