Job Description
Position: Physical Therapist(PT) - Home Health
Start Date: 9/8/2025
Duration: 13 weeks
Location: Littleton NH 03561
Shift Type/Shift Times/Hours/Day: Days8
Shift Notes: Monday - Friday; around 8a/9a start: 40 hrs/week
Weekend Requirements: None
On-Call Requirements: None

Traveler Policy: na
Dress Code: business casual no open-toe shoes
Non-billable orientation: 0hrs
OT Holiday multipliers: 1.25%
Assignment location: around the Littleton Berlin & Plymouth area

Submission Requirements
- 2 years of current experience
- age group experience: Peds to Geri
- The previous traveler preferred
- Home Health and/or Hospice experience
- OASIS experience
Profile Requirements:
- Work History
- Skills Checklist (pertinent to position) current to 1 year
- 1 Supervisory Signed Reference (from within the past two years of employment)
- License/Certifications - Must be uploaded with profile (NH license BLS - Facility will not pend License/Certs)
- BLS (AHA)
- New Hampshire
