Join us in making a difference at Home of Guiding Hands as we improve the quality of life for people with special needs. As a Case Manager you will provide oversight for the Independent Living (ILS) and Tailored Day programs. You will supervise employees and ensure that they have the necessary support to help clients achieve their goals. What Youll Be Doing:
Provide oversight of Independent Living and Tailored Day programs.
Responsible for the hiring training monitoring supervision coaching and evaluation of employees establishing clear expectations and fostering a culture of compliance and accountability.
Drive service utilization through staff engagement.
Ensure quality services by monitoring and ensuring delivery of services in accordance with the ID Team. Utilize customer service and problem-solving skills as needed to resolve issues.
Understand current local state and federal law and regulations and apply knowledge to ensure compliance.
Participate in audits conducted by monitoring bodies (SDRC and DDS).
Process referrals and facilitate matching new clients with current staff members.
Complete new client assessments and intakes.
Maintain client charts and authorizations.
Carry a caseload and provide direct client services as needed.
Coordinate group social activities by planning and leading activities.
Ensure accuracy and timely submission of payroll and billing data.
Participate in outreach to educate the community about available services.
Attend trainings and networking events to learn about best practices and form professional working relationships with industry professionals.

What Youll bring to the Role:
Required Qualifications & Experience
Education: High school diploma or GED.
Experience:
3 years of experience in a human services delivery system with at least one year in a comparable program; OR
A bachelors degree in a human services-related field.
Languages: The ability to read write speak and understand English. Bilingual in Spanish is strongly preferred.
Driving: A valid California drivers license with a safe driving record and at least two years of driving experience. Must maintain current and adequate auto insurance.
Other Requirements: Must be 18 years of age or older and able to pass a criminal record clearance from the DOJ.
Leadership: Demonstrated ability to provide staff training supervision and planning.
Communication: Excellent interpersonal and communication skills with the ability to directly address areas of concern.
Organization: Highly organized with the ability to follow through and consistently complete tasks by their deadline.
Problem-Solving: The ability to exercise sound judgment within general policy guidelines.
Technical: Proficient in Microsoft Office programs including Word Outlook and Excel.
Mindset: A passion for excellence through teamwork positive change and communication.
Physical Requirements/Work Environment
The work environment for this position involves working in an office setting for the majority of the time and on a less frequent basis meeting with clients in their homes and the community. While performing the duties of the job the employee is regularly required to see talk hear walk bend stoop sit stand reach and lift objects weighing up to ten pounds repeatedly during a shift.
