The Independence and Wellbeing Service was established to prevent reduce and delay demand on services. As part of this service the Access Team acts as Adult Social Cares first point of contact (for people over 18 years) Under the leadership and management of theTeam Manager to work within a team the Senior Access Assessor will ensure that the team continues to be the first response for Adult Social Care receiving referrals information requests and concerns from the public other professionals and external agencies via a variety of media.
The Role:
The Senior Access Assessor role is toembed and support in the day to day management of the team to ensure there are effective processes in place to gather information and identify and initially assess peoples presenting social care needs through discussion and to decide best next steps. Best next steps could be information and advice signposting reablement or assessment including responding to safeguarding alerts. The information will be recorded on computerised records.
Working Pattern:
The working pattern for this role is Monday to Friday with core hours 8.45am to 5.15pm Monday to Thursday and 8.45am to 4.45pm on Friday although there can be some flexibility. The Team works in a hybrid manner with some days in the office but mostly work from home.
