Qualifications:
- Education: Bachelor of Science in Nursing (BSN) preferred.
- Licensure: Current and unencumbered Registered Nurse (RN) license in the state of practice.
- Experience: A minimum of two years experience in a management or supervisory role within a home care or home health setting is highly preferred.
- Technical Skills: Strong knowledge of accepted professional standards Medicare Conditions of Participation and federal state and local regulatory requirements.
- Soft Skills: Exceptional leadership innovation mentorship communication and interpersonal skills. Ability to serve as a key liaison between patients families and staff.
Key Responsibilities:
- Lead and manage the delivery of patient care services ensuring compliance with all regulatory standards.
- Provide supervision mentorship and support to nursing staff to promote professional development and optimal patient outcomes.
- Collaborate with interdisciplinary teams to create and implement personalized patient care plans.
- Ensure precise and timely documentation of patient care and progress.
- Conduct regular audits of patient records and care plans for quality assurance.
- Serve as a communication link between patients families and healthcare providers to ensure seamless care coordination.
- Engage in continuous quality improvement initiatives and professional development activities.
