About the role
Deliver patient-centred short-term Occupational Therapy rehabilitation interventions within the inpatient acute care setting. You will be providing patients with early access to therapy aiming to improve patient outcomes reduce hospital length of stay at times minimising the need for inpatient rehabilitation.
Lead complex discharge planning using advanced clinical reasoning and standardised assessments (e.g. FIM GAS) to support optimal patient outcomes.
Collaborate closely within a multidisciplinary team (Acute Allied Health and medical teams as well as Rehabilitation MDT) to design and implement evidence-based goal-oriented treatment plans.
Drive quality improvement and service evaluation initiatives contributing to data collection outcome measurement and continuous service development.
Provide expert clinical advice and leadership across the Occupational Therapy department and wider hospital network supporting best practice in acute rehabilitation.
About you
Youre a qualified and AHPRA-registered Occupational Therapist with a current Class C Drivers License and a strong commitment to the Mission and Values of St Vincents Health Australia.
Bring extensive post-graduate experience in acute and sub-acute hospital settings managing complex care needs through advanced rehabilitation approaches in acute environments.
Confident in applying clinical reasoning and exercising sound professional judgement in both routine and complex discharge planning within a multidisciplinary team.
A clear and effective communicator skilled in supervision and leadership with a proven ability to build strong relationships with patients families colleagues and students.
Committed to continuous service improvement and innovation with experience leading quality initiatives research projects and contributing to service development
