Service Coordinator

Reporting to:Hub Manager

Base: Clonakilty Day Services

Hours: 35 hours per week

Contract: Permanent

Overall Purpose of Position

The purpose of this post is to develop and run a service for adults in West Cork who require individualised and significant support due to their level of support need. This includes managing the day-to-day service including programme planning scheduling staff supervision and identifying staff training needs and co-ordinating same. It includes ensuring each individual has an up to date and relevant person centred plan which informs their daily experiences and progresses them in independence within their respective and or chosen communities.


The Coordinator is responsible for the design and planning of this service under the guidance of CoAction Management. This is an evidence-based service centred on increasing the skills and independence of individuals accessing it. The Coordinator is responsible to track progress and empirically validate all aspects of the service by demonstrating the measurable benefits to the individuals accessing this service.


35 hours per week. FTE is 39 hours per week.

Remuneration:

The salary for this role will be based on the CoAction 2024 salary scale and will be 34383 t0 49101 per annum pro rated.

Please note this pay scale is subject to increase is 2025 in accordance with the recent WRC agreement towards enhanced pay adjustments in Section 39 organisations.
