Duties:
- Provides initial and ongoing agency contact with individuals agencies vendors and institutions inquiring about the agencys services and arranges for the placement of appropriate home health aides.
- Supports communication with other administrative staff to coordinate care for the agency clients.
- Serves as the lead scheduler responsible for complex cases and interacts with the APCS billing team members as it relates to service schedule.
- Supervisory Responsibilities: Trains new Administrative Service Coordinators on EMR system scheduling processes and acts a mentor.
- Essential Duties: Include but are not limited to:
- Receives and screens requests for home health aide services.
- Contacts families agencies or other referral sources to obtain supplemental information regarding potential clients needs.
- Explains agency services and policies customers.
- Refers service requests outside the scope of APCS services to the appropriate community resources which may best meet the inquirers needs.
- Requests information regarding the clients health insurance coverage when appropriate and assists with keeping the client advised and updated on the status of their insurance coverage.
- Follows a caseload of clients determined by the Office manager.
- Coordinates client coverage issues with other coordinators.-Maintains information about each client including their initial referral billing information the aides schedule and updates notes on each client in APCS EMR.
- Schedules aides for the assigned caseload and enters schedules.
- Notifies the case manager of aide changes which necessitate supervision.
- When service is terminated ensures all parties are notified and invites chart closure procedure.
- Documents aide-specific work history/ problems tracks call outs tardiness and makes recommendations to Business office coordinator for employee discipline and education as needed to discuss performance issues.
- Communicates aide status requests notifies office Manager via e-mail with details of the concern and recommended actions to be taken.
- Notifies billing office of changes in billing status and notes in EMR.
- Coordinates with the case managers and Clinical Manager to promote efficient response to clients request for service.
- Participates in agency planning activities and performance improvement activities as needed.
- Obtains report from evening and weekend on-call staff and follows-up on issues not resolved during off-hours.
- Gives report to on-call staff in the evening and on Fridays in preparation for the weekend.
Required Skills & Experience:
- At least one years experience in Home Care particularly with scheduling or related duties.
- Experience with customer service.
- Must possess exceptional oral communication skills.
- Organizational and computer skills.
- Ability to multi task and operate multiple software programs at one time.
- Able to relate to the public and others courteously and effectively by telephone.
