Job Description

Home Health PT working with a great team

- Organize and conduct medically prescribed physical therapy treatment programs for adult and pediatric patients.
- Evaluate new patients and set measurable treatment goals for functional outcomes.
- Establish and implement appropriate treatment programs tailored to patient needs.
- Administer diagnostic tests (muscle nerve joint functional ability) to assess impairment and prognosis.
- Carry out treatment modalities and regularly re-evaluate and modify treatment plans based on patient progress.
- Document patient care education and communication with other caregivers objectively and analytically.
- Communicate effectively with medical and allied health personnel regarding patient progress needs and discharge planning.
- Provide instruction in therapeutic procedures for patients and families to continue independently.
- Assist in training new personnel as requested by management.
- Provide guidance and direction to Physical Therapist Assistants Aides and students.
- Maintain departmental policies procedures quality assessment improvement initiatives and safety standards.
- Attend required meetings and participate in professional development activities.
- Maintain daily statistical records of patient activity and billing charges.
- Perform additional work-related duties as assigned.
- Consistently provide service excellence in line with Cape Cod Hospitals CARES values: compassion accountability respect excellence and service.

For Physical Therapy Clinical Specialist (S19T):
- Serve as a clinical resource and specialist for rehabilitation staff in a focused area of expertise.
- Plan conduct and supervise specialized treatment programs.
- Assist management in planning developing and supervising the specialty clinical area.

Required Qualifications:
- Bachelors degree in Physical Therapy from an accredited program.
- Licensed to practice Physical Therapy in Massachusetts.
- Current BLS certification.
- For Clinical Specialist: Documentation of Specialist Certification.
- Ability to read write and communicate effectively in English.
