The Native American Community Clinic is dedicated to addressing health disparities within the urban Native American community of the Twin Cities. Our mission is to promote holistic health and wellness for Native American families through comprehensive healthcare services.
SUMMARY:
The Community Health Worker (CHW) role serves as a link between community members and clinical care teams to improve access to care and reduce health disparities. This role involves building relationships conducting outreach providing culturally appropriate education and connecting individuals to needed medical behavioral health and social resources. CHWs support individuals in navigating complex systems overcoming barriers to care and managing chronic conditions while empowering them to take an active role in their health.
DUTIES AND RESPONSIBILITIES:
Outreach & Engagement
o Build trusting relationships with patients families and staff.
o Conduct outreach and support during events sponsored by organization.
o Identify patients in need of support and conduct follow-up.
Navigation & Resource Connection
o Link individuals to resources such as primary care behavioral health and dental care.
o Provide follow-up to ensure needs are met and barriers are addressed.
Health Education & Support
o Deliver culturally appropriate health education on topics such as preventive care chronic disease management and healthy behaviors.
o Support individuals in setting health goals and developing plans.
o Conduct phone outreach to individuals that have gaps in clinical care or were recently in the hospital or emergency room.
Documentation & Reporting
o Document all patient engagements in organizations electronic medical record.
o Track outreach activities services provided and outcomes.
o Support the tracking of grant activities patient contacts and other program activities.
o Support clinical care activities such as patient forms and referrals.
Collaboration & Advocacy
o Work closely with medical care teams and community partners to coordinate care and resources.
o Advocate for clients needs within health and social service systems.
o Participate in program meetings trainings and community coalitions.
o Assists with other duties as assigned.
SUPERVISORY RESPONSIBILITIES: This role has no supervisory responsibilities.
QUALIFICATIONS:
High school diploma or equivalent.
Completion of an accredited Community Health Worker training or certification program.
At least one year experience in health care or related field.
Strong communication active listening and interpersonal skills.
Knowledge of the community culture and available resources.
Ability to work independently and as part of a team.
