Job Overview/Summary:
We are seeking a compassionate and experienced Senior Health Primary Care Provider to join our growing clinical team. This role involves managing a diverse patient panel providing evidence-based acute chronic and preventive care across all ages and collaborating with both in-office and virtual care teams to deliver human-centered primary care. The ideal candidate will bring clinical expertise strong communication skills and a passion for transforming healthcare delivery.
Responsibilities:
Manage a panel of patients with varied healthcare needs delivering acute chronic and wellness care.
Conduct in-person visits in clinics or testing centers and occasional telehealth visits.
Participate in ongoing clinical learning through weekly Clinical Rounds and other educational modalities.
Collaborate daily with in-office teammates during huddles and with virtual clinical teams.
Perform in-office procedures aligned with your clinical training and scope of practice.
Utilize technology effectively to enhance care quality and efficiency.
Primary Skills:
Strong clinical proficiency in evidence-based primary care.
Ability to care for patients across all ages (pediatrics to geriatrics).
Excellent communication and interpersonal skills for working with diverse populations.
Adaptability in dynamic healthcare settings including telehealth and team-based care.
Secondary Skills (Good to Have):
Experience with in-office procedures.
Familiarity with value-based care models and population health.
Comfort with technology-enabled healthcare delivery.
Qualifications:
Enrolled in or graduate of an accredited Family Medicine or Medicine/Pediatrics residency program.
Minimum 2 years of outpatient primary care experience within the last 5 years.
Board certified in Family Medicine or Medicine/Pediatrics or board eligible with certification planned within 1 year of start date.
Valid State of Arizona medical license (must be obtained prior to start date).
