Job Description
Nurse Practitioner - Palliative Care
Join a dedicated team at a community-based nonprofit facility in Northwest Ohio providing compassionate care in palliative care hospice and geriatrics. This position offers a unique opportunity to work within Lima Memorial Hospital combining inpatient and outpatient care while making a difference in the community.
Job Responsibilities:
- 75% Inpatient Palliative Care; 25% Outpatient Palliative Care
- Deliver comprehensive palliative care services at Lima Memorial Hospital
- Conduct patient consultations for symptom management and advanced care planning
- Lead sensitive discussions with patients and families about terminal diagnoses
- Perform inpatient rounds for palliative care and hospice patients
- Manage outpatient palliative visits via various departments including Telehealth
- Collaborate with hospital specialists to align care plans effectively
- Provide independent care in a fast-paced hospital environment
- Maintain accurate documentation in Meditech and eClinicalWorks
- Act as a clinical expert in palliative care educating staff on best practices
- Manage up to 6 patient encounters per day
Qualifications:
- Board Certified Nurse Practitioner
- Experience as an advanced provider in an acute care setting or with palliative care and hospice
- Clean malpractice record and active license
License and Certification Requirements:
- Ohio NP license (or willingness to obtain)
- DEA registration (must be completed before starting)
