The Opportunity:
The Social Worker reporting to the Director of Care is a key member of the interdisciplinary care team collaborating on the delivery of care to meet resident and family needs. The Social Worker provides a range of services to and on behalf of residents their families the staff and the community at large utilizing a holistic framework.
The Social Worker provides services which include a wide range of planned interventions financial/legal assistance adjustment support consultation counseling education and referral following a comprehensive psychosocial assessment.
What Is In It For You:
- Ensures all newly admitted residents have a comprehensive psychosocial assessment which includes an examination of psychological social and spiritual well-being.
- Contacts resident and family following admission to set up an initial interview session.
- Engagement establishes a rapport with the resident and/or family to enter a helping relationship.
- Conducts interviews establishing its purpose and communicating empathy support understanding genuineness trustworthiness and respect to develop a supportive and therapeutic relationship.
- Utilizes various techniques including reflecting paraphrasing clarifying informing summarizing assuring and interpreting to gather information.
- Outlines the boundaries of the therapeutic relationship with discussion of Social Worker availability confidentiality and sharing of personal information.
- Attempts to establish a therapeutic relationship of trust where the resident and/or family feel comfortable in sharing personal information related to the residents history which will form the foundation of the psychosocial assessment.
- Life Review including early life history education family of origin work relationships and interests.
- Identification of major life events: achievements losses moves illness deaths and the coping style utilized by the identified resident and key family members.
What You Will Need:
- Must be registered as a Social Worker under the Health Professions Act 2000.
- Graduate of a recognized school of social work with a Bachelor of Social Work degree.
- Experience in working with Geriatrics.
- Familiarity with the multi-disciplinary team concept skill in teamwork and ability to work effectively in a multi-disciplinary setting.
- Ability to initiate and direct own departmental activity.
- Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one.
Wage Levelling Salary: $42.27 - $52.81 per hour
Salary placement based on qualifications and experience.
