Main Purpose of Job

This Coordinator of Special Programs II HS position is a part-time (32 hours/week) position and will provide ongoing screening and referral Care Coordination Services and collaboration between consumers and community partners such as the Recovery Wellness Center DUI/Drug Court Probation/Parole Coordinated System of Homeless Services Residential Treatment Providers Transitional Programs and the Behavioral Health Treatment Center and other related services to consumers and their families with substance use and mental health issues in Howard County who seek to recover from these disorders. This position also requires grant monitoring experience which includes ongoing data and progress reporting in the designated electronic health record (EHR) system. This position will provide program grant monitoring of program funds which may include monitoring contracts to ensure compliance with performance standards expense limits and service provisions. This position will also conduct grant monitoring/site visits bi-annually and submit audit documentation to the Behavioral Health Administration (BHA)/Bureau Director by due dates. Additionally this position is designed to assist individuals with substance use as they move from one level of service to another with the goal of providing smoother and stronger connections and support between treatment agencies and community support networks. It requires knowledge of treatment and community services as well as Care Coordination and Case Management Services. Multicultural experience training and knowledge regarding how to refer and connect consumers from many populations to services is critical in order to assist in transitions back to communities. This position will also be responsible for conducting outreach to community and behavioral health providers as needed.

POSITION DUTIES

Provides Direct Patient Services
Provides a wide variety of specialized and varied direct patient services as outlined by current performance measures designated by the Behavioral Health Administration. Provides outreach to community and behavioral health providers (outside of site visits) regularly.

Develops Recovery Plans
Works collaboratively and develops the appropriate relationship with the patient to develop a recovery plan that addresses the problems unique to the individual with measurable criteria for completion and improving the quality of life.

Prepares Reports and Evaluations
Prepares written reports and evaluations as required by state COMAR agency Policies and Procedures and MDH/BHA. This includes grant monitoring related duties: data reporting billing etc.
Case Management and Care Coordination
DutiesAssisting clients in applying/obtaining housing insurance benefits for DSS/SSA SSI/SSDI Medical Services GED assistance employment services replacement identification cards transportation cognitive testing triaging community services conducting surveys etc. Provides BHA mandated Care Coordination and monitoring for all Howard County patients referred to inpatient treatment to ensure that once they are discharged they are transitioned successfully to the next level of care. Monitors the Medicaid
SUD rates for recovery housing.

Grant Monitoring
Grant monitoring of program funds for SCC and MDRN which may include monitoring contracts to ensure compliance with performance standards expense limits and service provisions. Data reporting monthly and quarterly for SCC and MDRN. Conducting grant monitoring/site visits bi-annually and submitting audit documentation to the Behavioral Health Administration (BHA)/Bureau Director. Reviewing and preparing invoices associated with contracts in preparation for payment.

Other Duties as Assigned
Possesses a broad understanding of HCHD services and can refer to other programs. Participates in necessary drills and exercises responds to emergencies as appropriate keeps abreast of policies and procedures in the program bureau and department. Completes all required trainings. Participates in quality improvement efforts within the program.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:
1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.
2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess the following:
  • Experience with Electronic Health Records (EHR).
  • Experience with case management and maintaining a case load.
  • Experience with conducting outreach to community and behavioral health providers.
  • Experience with grant monitoring which may include monitoring contracts to ensure compliance with performance standards expense limits and service provisions.
  • Experience with completing client referrals and coordinating treatment services.
