Summary

The Freedom Navigator is responsible for outreaching and engaging individuals that are Justice and Opioid Involved and will be experiencing homelessness. The target population is mostly likely to experience unsheltered homelessness. Outreach will be mainly outside in emergency shelter or in correctional settings in Maricopa County. The Freedom Navigator will connect the client to appropriate care and services such as shelter employment healthcare and financial assistance programs to assist the client with resolving their homelessness and reducing the likelihood of returning to the justice system.

What You Will Do To End Homelessness

  • Support and outreach a caseload of 15-20 clients experiencing homelessness and coordinate services for justice-involved individuals who have a history of opioid involvement/OUD to link them to care.
  • Partnering with Maricopa Countys Correctional Health Services and Maricopa County Department of Public Health to understand how to collaboratively meet the needs of those justice-involved individuals who have a history of opioid involvement/OUD and link them to care.
  • Determining and documenting processes/systems to identify the target population and their care needs.
  • Tracking individuals referred to care and determining a process/system to do so.
  • Guide the client in obtaining necessary documents for housing such as: Seriously Mentally Ill (SMI) evaluations State Identification Birth Certificate documentation of disability etc.
  • Report program progress and results to Maricopa Correctional Health and other appropriate partners.
  • Connect client to services that are appropriate based on the indicated needs.
  • Conduct intake and assessments (VI-SPDAT and SPDAT) as needed.
  • Collect client demographic information and obtain appropriate releases of information.
  • Facilitate warm hand-off into needed service programs.
  • Screen for eligibility for SOAR and employment services providing referrals when appropriate.
  • Enter information into the Homeless Management Information System.
  • Provide services in a manner congruent with harm reduction progressive engagement low-barrier eligibility trauma informed care and national best practices.
  • Engages clients using best practices including motivational interviewing conflict resolution and crisis de-escalation.
  • Works closely with HSC staff and partners providing entry point in-reach outreach engagement and housing services as needed.
  • Maintains adherence to the HSC and Campus policies attends yearly updates and any specific trainings established for their department. Promptly reports any violations to their immediate supervisor.
  • Attends required staff meetings and agency events.
  • Adheres to policies regarding working hour
  • Other duties as assigned.

Youre A Good Fit If You Have The Following:

  • Minimum of 2 (two) years experience in intensive case management and outreach preferably with individuals experiencing homelessness. Training in VISPDAT/SPDAT trauma informed care motivational interviewing preferred. Experience working with homeless or other vulnerable populations.
  • Must be able to effectively communicate with clients from diverse backgrounds including those who are experiencing mental health challenges substance use trauma and medical barriers.
  • Must be able to work in a fast-paced environment; work as part of a team; use sound judgement; demonstrate personal responsibility and integrity; show initiative and an ability to work interconnected. Sensitivity to cultural and lifestyle diversity is essential.
  • This position is 75% outdoors. Applicant must be a willing to work in high heat and inclement weather situations on the streets.
  • Effective written and verbal communication skills (Spanish a plus).
  • Valid Arizona State Drivers license as this position will have a significant amount of travel involved.
  • Visual acuity sufficient to maintain system of records and reports containing computer-generated and handwritten documents.
  • Ability to handle sensitive material maintaining the highest level of confidentiality and displaying sound judgement.
  • Mobility sufficient to conduct regular duties within a normal office environment.
  • Ability to lift up to 20 pounds in order to safely manage office supplies and equipment.
  • You have previous experience working with homeless populations.
  • Experience working in areas of crisis with vulnerable populations.
  • Multi-lingual (Spanish a plus).
  • You adapt well to a fast-paced environment.
  • Like working outdoors away from a desk.
  • Fingerprint Clearance Card and Background Check are required for employment
