Description

ECM Lead Care Coordinator *** Urgently Hiring

Monday to Friday : 8:30 am to 5:00 pm

Salary/Non-Exempt : $24.03 to $25.00 (hourly)

** Hybrid - Office/Field and Home

*** Majority of our positions are remote but services are provided either hybrid or in person

SHIELDSmission is to develop deliver and evaluate culturally sensitive comprehensive service models that empower and advocate for high-risk families in South Los Angeles.

Summary:

Responsible for administering Enhanced Care Management Services to Medi-Cal beneficiaries confronting complex health and social needs in South Los Angeles who are enrolled in the Healthy Start Program.

Primary Duties:

  • Works in collaboration with the Licensed Clinician to conduct comprehensivemember needs assessments in the areas of physical health; mental health; SUD; oral health; trauma- informed care; social supports; housing; vocational/employment; wellness; and referral and linkage to community-based services and supports.
  • Collaborates with the Licensed Clinician to develop the Individual Care Plan/Health Action Plan and implement the Individual Care Plan/Health Action Plan.
  • Assist with development of Family Service and Reproductive Life Plan
  • With permission coordinates/advocates on behalf of client/member with health care professionals (e.g. PCP)
  • Provide evidence-based home visiting care coordination/case management
  • Provide referrals and link clients to auxiliary services and assisting clients and their families with accessing appropriate treatment.
  • Conducts outreach and engagement activities to enroll Managed Care Plan membersto the ECM program. Outreach and Engagement consists of phone calls mailed information and field visits.
  • Accompaniesclients/members to office visits as needed and appropriate.
  • Evaluates member progress and update goals.
  • Utilizes evidence-based practices such as Motivational Interviewing Harm Reduction Techniques and Trauma- Informed Care principles.
  • Works collaboratively with hospital staff to support Transitional Care Planning
  • Compile proper client documentation and prepare reports in compliance with SHIELDS policies and procedures.
  • Responsible for collecting required data maintaining participant records.
  • Attend in-service and outside training as required.
  • Participate in multidisciplinary case conference and staff meetings as assigned.
  • Maintain a working knowledge of existing local private and public community programs the services they provide and their eligibility requirements.
  • Maintain good working relationships with local linkage and referral agencies.

Qualifications

Requirements

  • Two (2) years of experience in healthcare case management homeless services or working with vulnerable populations or a related field.
  • High School Diploma or GED
  • Know and comply with Agency policies and procedures HIPAA Department of Mental Health policies and documentation guidelines and other state/federal regulations relating to service programs for adults with a history of mental illness.
  • Basic advocacy skills.
  • Present ideas information and viewpoints clearly both verbally and in writing.
  • Efficiently use the personal computer to include a variety of word processing software.
  • Experience usingEMR/EHRs/Care Management software
  • Utilize analysis experience and judgment to make decisions.
  • Demonstrate commitment to team objectives and SHIELDS For Families philosophies.
  • Adapt to changing needs by acquiring new skills and knowledge.
  • Provides services within a culturally sensitive ethnic informed model of practice and is respectful to the community and clients.
  • A valid California drivers license is required for this position as driving is an essential function of the role and is required for field work client crisis response and emergencies. Avalid car insurance and registration is required.
