Job Description

Position: Case Manager

Program/Department: Transitional Living Community (TLC)

Reports to: Program Director

Work Location: East New York Brooklyn NY 11207

Job Type: Full Time - 35 hours per week

Shifts Available: Day Shift - Tuesday -- Saturday and Sunday -- Thursday

Salary Range: $50000 - $50000

Position Summary:

BCSs Transitional Living Community (TLC) a 30 bed section of the Brooklyn Womens Shelter in East New York Brooklyn was established in 1989. TLC provides homeless low-income mentally ill women with temporary shelter and assistance to secure permanent housing. TLC recruits its clients primarily from the assessment unit of the Brooklyn Womens Shelter. Staff conduct comprehensive assessments to identify women with severe and persistent mental illness many also having serious personal social medical and economic problems. A principle feature of the TLC program is the milieu treatment in which women are helped to practice the skills they will need to live successfully outside the shelter system including pre-vocational preparation.

TLC is a transitional living community for unhoused women living with a mental health diagnosis. The Case Managers work with the team to coordinate social service tasks. Case Managers work with other staff to ensure that our clients are prepared for permanent housing by developing rapport with clients streamlining intake procedures scheduling regular case management meetings and coordinating with service providers (mental health medical care coordinators ACT teams etc.); as well as helping clients to build their personal social and economic safety nets.

Responsibilities:

  • Provide consistent high quality program service delivery to all client populations in accordance with funder and regulatory requirements; best practices; BCS policies and Core Values; and Code of Conduct for Custodians of People with Special Needs. Serve as a role model to all stakeholders.

  • Schedule regular meetings (at least twice per month) and engage clients to ascertain needs using a milieu approach

  • Assist all clients with self-administered medication by providing access to all prescribed medication at appropriate times encouraging correct use of medication logging medication taken and assuring proper storage in locked area.

  • Complete weekly progress notes clients psychosocial histories and treatment plans/reviews using database

  • Complete assessments and scales including biopsychosocial assessments

  • Update client demographics and other data in CARES and database in a timely manner.

  • Ensure Client Adhere to COVID standards and requirements.

  • Complete case notes and quarterly Service Plans in CARES database and immediately report all placements to Supervisor for entry into CARES database within 24 hours.

  • Assist with facilitating admissions and initial intakes as needed

  • Attend trainings as assigned by BCS DHS and TLC

  • Schedule medical appointments for routine and urgent referrals

  • Schedule appointments with TLC psychiatrist and provide follow-up to obtain lab results and appointment summaries

  • Interact with clients in therapeutic manner and attend to their needs and requests according to program policies

  • Link clients to HRA programs SSI Public Assistance mental health clinics and other community resources

  • Work with Housing Specialist to prepare materials for submission to city agency for housing placements from your caseload with a focus on long term clients at TLC

  • Ensure that service plan goals are being updated and met

  • Prepare for case conferences and report pre-placement activities to Supervisor

  • Deliver services and engage with clients with a non judgemental harm reduction approach

  • Other tasks as may be required

Qualifications:

  • Required: BSW/BA in Human Services related field

  • One-year experience working with shelter and homeless and/or mentally disabled populations preferred

  • Prior training experience in CARES - Collaborative Homeless Management Information System database is helpful

  • Basic knowledge of MS Word Google Suite APRICOT and other database programs

  • Ability to take initiative multitask and work independently in a solutions focused approach

  • Previous experience working in residential direct care preferred
