Worksite Location: Los Angeles General Hospital -
1200 N State St Los Angeles CA 90033
Monday to Friday 8:30 am to 5:00 pm (flexible based on program/client needs)
Salary: $28.84 to $33.65 (hourly) Non-Exempt
Hybrid Remote and Office/Field
*** Majority of our positions are remote but services are provided either hybrid or in person

SHIELDS mission is to develop deliver and evaluate culturally sensitive comprehensive service models that empower and advocate for high-risk families in South Los Angeles.

Position Summary:

The POSC Navigator works in collaboration with hospital social work staff to support prenatal and postpartum individuals through bedside engagement and home visits. This role includes developing Plans of Safe Care (POSC) conducting assessments providing education on maternal and infant health and connecting families with community resources. The POSC Navigator uses a compassionate family-centered approach to build trust ensure timely documentation and maintain positive relationships with referral agencies. Occasional after-hours response may be required.

Key Responsibilities:

  • Complete participant assessments and develop individualized POSCs
  • Provide in-home support and education on parenting health and safety
  • Coordinate referrals to appropriate services including SUD treatment and home visitation
  • Maintain accurate and timely documentation in electronic systems
  • Participate in case conferences staff meetings and training sessions
  • Ensure strong collaboration with hospitals social workers and community agencies

Qualifications

Qualifications

Basic advocacy skills.
Present ideas information and viewpoints clearly both verbally and in writing.
Efficiently use the personal computer to include a variety or word processing software.
Utilize analysis experience and judgment to make decisions.
Demonstrate commitment to team objectives and SHIELDS For Families philosophies.
Adapt to changing needs by acquiring new skills and knowledge.
Provides services within a culturally sensitive ethnic informed model of practice and is respectful to the community and participants.
Experience with substance abuse and an under-served population is required SUD certificate preferred.
Bachelors degree required.
Bilingual (English/Spanish) required.
Experience working with maternal health preferred.
A valid California drivers license is required for this position as driving is an essential function of the role and is required for field work client crisis response and emergencies. A current automobile insurance is required.
