Job Description

The South West Aboriginal Medical Service (SWAMS) is an Aboriginal Community Controlled Health Service based in Bunbury which provides a variety of health services to Aboriginal people in the South West Great South Upper wheatbelt and Peel Regions of Western Australia. SWAMS delivers a wide range of Services including: GP Clinics Primary Health Care Counselling NDIS and community host various programs. SWAMS has a strong growth strategy through partnership opportunities and future community development.

About the role

This role will see you mentoring leading and developing a team of access workers that provide a very important service to the community. The primary purpose of this position is to in accordance with the clients assessed needs manage access to disability services for identified clients. The position will review ongoing service needs in the delivery of services ensuring quality timeliness and effectiveness of service delivery. Although this role is based out of our Bunbury office there will be opportunities to travel throughout the South West Great Southern and Wheatbelt (including overnight stays). This role is varied and will include:

  • Provide practical and innovative leadership in the design implementation and evaluation of access pathways for aged care and disability
  • Identify a range of services that enhance inclusion for Aboriginal people who are residing in their own homes within the community
  • Collaborate with other service providers to ensure effective service delivery and increase community networks
  • Assist in the planning implementation review and evaluation of programs
  • Liaise with assessors and stakeholders to ensure service delivery is tailored to meet a clients needs.
  • Day to day management of a team including rostering timesheet management performance management team building mentoring.
  • Develop an onboarding and offboarding procedure which details service delivery access and referrals processes
  • Coordinate team to undertake risk functional and psychosocial assessments as appropriate for the client in conjunction with family/carer to identify priorities for transition care and long-term care goals.
  • Facilitate and coordinate meetings and sessions between participant/s relevant stakeholders remote community connector and customer staff that address access requirements and plan build/s.
  • Facilitate and coordinate local linkages with the local health clinic community school and other community-based service organisations working in and alongside (as relevant) access and customer staff.
  • Liaise with the clients carers/family to ensure access requirements meet the individual needs of the client

About you

Along with being passionate about disability services and the health outcomes of our Aboriginal community you will bring with you the following:

  • Demonstrated knowledge and experience working in the Community Services sector including aged care and/or disability services
  • Previous leadership experience
  • Demonstrated skill and/or ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander persons.
  • An understanding of the health and social issues impacting upon Aboriginal people
  • Well-developed understanding of care delivery in community care setting and within continuous improvement
  • Excellent organisational and problem solving skills
  • Demonstrate respect and understanding of client issues including cultural appropriateness and anti-discrimination
