Job Description

Part Time / 30 Hours / BWH Boston Main Campus

Plays a key role in partnership with an interdisciplinary care team in providing navigation support to patients with Substance Use Disorder (SUD) seen in the Emergency Department to identify the next step in care after discharge and to address any barriers to follow up.

Will meet with patients in-person while they are in the Emergency Department and work with the care team to assist patients in finding and making referrals to ongoing SUD treatment mental health services and other community resources related to social determinants of health.



Job Summary

Essential Functions (Key Roles & Responsibilities)
Work with an interdisciplinary care team of Emergency Department and Addiction Medicine clinicians to assist with patients transition to further addiction treatment after discharge; take direction from team medical and clinical leadership as appropriate to optimize patient care.
Proactively monitor patients with SUD who enter the Emergency Department to make suggestions to the responding clinician about accessing SUD services like consultation from the Addiction Consult Team or referral to the onsite outpatient Bridge Clinic.
Make referrals to the full continuum of addiction treatment services inside and outside the system including but not limited to outpatient treatment opioid treatment programs ATS and CSS.
Maintain an up-to-date understanding of local SUD and related resources and best practices for referral.
Provide assistance to individuals/families in accessing a variety of public services that may address barriers to patient follow-up with addiction treatment including financial and medical assistance and directly help to meet needs for food shelter transportation and clothing.
Complete follow up phone calls to patients to ensure a successful transition to the treatment organization in the disposition plan.
Work as part of a Resource Specialist care team which may include cross coverage in other SUD care settings onsite like the inpatient Addiction Consult Team or the outpatient Bridge Clinic.

High School Diploma or Equivalent required or Associates Degree Related Field of Study preferred
