Position Title:Behavioral Health Patient Care Coordinator
Supervisor:Director of Behavioral Health
Location: Onsite
Hours:Monday - Friday 8:30am - 5:00pm
PRIMARY PURPOSE
The Behavioral Health Patient Care Coordinator plays a vital role in supporting the smooth and efficient operation of the Behavioral Health team. This position is responsible for a wide range of administrative and clinical support tasks ensuring that patients receive timely and high-quality care. The Coordinator works under the general supervision of the Director Behavioral Health and has significant patient contact requiring strong communication and organizational skills.
ESSENTIAL FUNCTIONS
Scheduling and Coordination:
- Schedule new patient appointments and follow-up visits for all BH patients.
- Manage and block BH provider schedules to optimize patient access.
- Coordinate telehealth appointments ensuring seamless connectivity for patients and providers.
- Manage insurance pre-authorizations for medications and follow up with pharmacy.
Patient Communication and Support:
- Complete phone screens with BH referrals to gather necessary information.
- Triage Medical Provider warm hand-offs.
- Provide patient reminders about appointments and co-payments/balances.
- Communicate/transfer patient questions to BH staff when appropriate.
- Monitor and respond to the BH email in a timely and professional manner.
- Mail No Show and Missed appointment letters
Record Management and Billing:
- Register patients and verify/update demographic and insurance information in the EMR.
- Manage patient information in EMR ensuring accuracy and completeness.
- Responsible for submitting billing in a timely manner.
- Ensure timely and accurate entry of billing information in the EMR.
Resource and Office Support:
- Maintain and organize the Behavioral Health Resource Library ensuring materials are current and accessible to clinical staff
PHYSICAL DEMANDS
- Considerable use of computer and telephone.
- Some lifting of supplies and equipment up to 50 lbs.
- 25% walking or standing 75% sitting at a desk.
- Category III re: exposure to blood bodily fluids and communicable diseases.
QUALIFICATIONS
- High school diploma or equivalent required.
- Associates or Bachelors degree preferred.
- Previous office experience (3-5 years) in medical or mental health practice desirable.
- Knowledge of medical terminology and insurances.
- Must be reliable and punctual.
- Must have attention to detail.
- Must be professional and customer-service oriented to deliver quality care.
- Ability to work both independently and with a team.
- Professional verbal and written communication skills
- Ability to be responsive in working with a culturally-diverse patient population in a community based setting.
- Competence in basic computer skills including proficiencies in Microsoft Outlook excel and word. EHR experience preferred.
- Ability to independently coordinate multiple tasks is essential.
- Excellent interpersonal skills to promote cohesive team based care.
- Must have valid drivers license and satisfactory driving record.
- Bilingual Spanish speaking is a plus.
