Join UChicago Medicine Home Careas an RN Case Manager.

The Registered Nurse/Case Manager plans organizes and directs home care services and is experienced in nursing with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.

Essential job Functions/Responsibilities:

  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates patient nursing needs.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Develops a care plan which establishes goals and incorporates therapeutic preventive and rehabilitative nursing actions as well psychosocial support as needed. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
  • Monitors patient response to interventions.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
  • Prepares clinical notes and updates the primary physician on Plan of Care patient progress and assessments / reassessments as indicated in agency policy.
  • Communicates with the physician regarding the patients needs and reports and changes in patients condition; obtains/receives physicians orders as required.
  • Coordinates and communicates with other disciplines involved in care.
  • Communicates with community health related persons to coordinate the care plan.
  • Participates in on-call duties as defined by the on-call policy.
  • Ensures that arrangements for equipment and other necessary items and services are available.
  • Instructs supervises and evaluates Home Health Aide care provided every fourteen days.
  • Supervises and evaluates LPN care provided every thirty days.
  • Commits to one hundred percent (100%) patient and customer satisfaction by always exhibiting a courteous and helpful manner during interactions with others including patients families visitors physicians students and co-workers.
  • Other duties as assigned.

Qualifications:

  • Current state of Illinois nursing license as an RN.
  • Current CPR is required.
  • Maintains a full-time position within the organization.
  • One to two years of recent acute care experience in an institutional setting required.
  • One year of home health care experience preferred.
  • Excellent observation verbal and written communication skills problem solving skills basic math skills; nursing skills per competency checklist.
  • Possesses and maintains a valid drivers license and current automobile insurance and has the availability of personal and dependable mode(s) of transportation to conduct home visits.

Physical Requirements:

  • This position requires the ability to perform medium work exerting up to 50 pounds. Job frequently requires bending and lifting push/pull maneuvers and occasionally carrying items that weigh > 20 pounds. Jobs frequently require standing walking reaching (floor to waist; waist to shoulder; above shoulder) and talking. Fine motor control/precision is required for tasks such as IVs and medication management. Visual acuity and hearing to perform required nursing skills is required in addition to see information in print and/or electronically. This position does involve exposure to chemicals/disinfectants and bodily fluids. This job does include travel to patients homes and other work-related places of business so the ability to operate a motor vehicle is required.

Continuing Education Requirements:

  • Ingalls Home Care personnel are expected to participate in appropriate continuing education as may be requested and/or required by their immediate addition personnel are expected to accept personal responsibility for other educational activities to enhance job-related skills and abilities and as required by licensing standards.

UChicago Medicine is committed to transparency in compensation and benefits. The pay range provided reflects the anticipated wage or salary reasonably expected to be offered for the position.

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data reviewed on an annual basis. Compensation offered at the time of hire will vary based on candidate qualifications and experience and organizational considerations such as internal equity. Pay ranges for employees subject to Collective Bargaining Agreements are negotiated by the medical center and their respective union.
