Job Description

Job Title: Speech Language Pathologist (SLP) Skilled Nursing/Rehab

Location: All States USA (multiple facilities)
Employment Type: Contract Per Diem or Full-Time
Facility Type: Skilled Nursing Facility (SNF) or Rehabilitation Center

Position Overview

We are seeking compassionate and dedicated Speech Language Pathologists to provide therapy services in skilled nursing and rehabilitation facilities nationwide. The therapist will work with a diverse caseload including short-term rehabilitative and long-term care residents to assess diagnose and treat speech language swallowing and cognitive-communication disorders. Collaborate with interdisciplinary teams and help patients improve function independence and quality of life.

Key Responsibilities
  • Assess diagnose and develop individualized treatment plans for residents with speech language voice cognitive-communication and swallowing disorders.

  • Deliver evidence-based therapy to a mix of short-term (rehab/acute) and long-term care residents with complex medical needs.

  • Implement therapeutic interventions including speech therapy language therapy cognitive retraining and swallowing/feeding management.

  • Educate patients families and caregivers regarding strategies for improved communication and safe swallowing.

  • Perform and document comprehensive evaluations progress notes and therapy summaries using EMR systems as required.

  • Participate in multidisciplinary rounds care conferences and discharge planning.

  • Collaborate with other therapy disciplines (PT OT) and nursing to ensure holistic patient care.

  • Ensure strict compliance with regulatory and facility policies (Medicare/Medicaid and SNF guidelines).

  • Engage in facility team meetings and professional development as necessary.

  • Maintain a safe supportive and respectful environment for patients with varying medical acuity.

Required Qualifications
  • Masters degree in Speech Language Pathology from an accredited program.

  • Current active state SLP license (or eligibility to obtain in the respective state).

  • Certificate of Clinical Competence in Speech Language Pathology (CCC-SLP) preferred.

  • CPR/BLS certification.

  • Previous experience in skilled nursing facility inpatient rehab or mixed acute/chronic care settings preferred.

  • Familiarity with EMR/electronic documentation.

  • Effective communication organization and interpersonal skills.

  • Ability to work collaboratively within an interdisciplinary team.

  • New graduates welcome; prior SNF/rehab experience a plus.
