Job Description

Opportunity:

Westcoast Family Centers Society (Westcoast) is looking for a Clinical Coordinator to work 5 hrs per week in the Vancouver office starting in Mid-September. This position will report directly to Director of Programs & Clinical Services. This job posting is open to staff currently based in Vancouver office only.

Job Responsibilities:

  • Lead continued development and implementation of emerging programs (ie. Westcoast Wellness Centre and NEST) by setting and monitoring realistic measurable goals.
  • Oversee and monitor program use of platforms and record management such as Psychology today JaneApp and program specific tracking. This includes supporting clients staff and contractors with technical challenges ensuring all data is being recorded to standard and monitoring email accounts for these programs.
  • Streamline and conduct client intake and assessment processes to ensure clients meet program requirements while collaboratively setting reviewing and adjusting individualized goal planning for clients.
  • Create maintain and ensure accuracy of data logs and client profiles to track progress and outcomes effectively.
  • Generate detailed summaries and reports on program services client progress and areas for improvement upon request.
  • Identify operational and clinical challenges/needs and proactively propose and implement effective solutions or adaptations as needed.
  • Liaise and collaborate with community professionals (e.g. schools healthcare providers social workers) to ensure integrated wrap-around care for clients
  • Develop and maintain strong referral networks and relationships with external professionals and agencies for potential new clients
  • Facilitate clear and consistent interdepartmental communication and coordination to promote cohesive and effective service delivery
  • Oversee program budgets including forecasting tracking expenditures and ensuring resources align with program needs
  • Participate in case reviews and team meetings as required
  • Identifying and supporting quality improvement initiatives to enhance program effectiveness and client outcomes
  • Ensure compliance with agency policies regulatory requirements and ethical standards
  • Maintain confidentiality and professionalism in all interactions with clients families and team members
  • Work independently with strong organizational and time management skills to meet deadlines and program goals


Requirements:

  • BA in human services or related field and specific education/training on team management or supervision.
  • Two (2) years recent related experience (service coordination program management or program development) or an equivalent combination of education training and experience.
  • Proficient knowledge of Computer operating systems (MS Word Excel Office Customer Management Systems)
  • Current First Aid Level C or ability to obtain this within the first 3 months of employment.
  • Current Non-Violent crisis intervention certificate or ability to obtain this within the first 3 months of employment.
  • Pass Criminal Record Check.
