Job Scope: The Occupational Therapist functions to provide assessment treatment and recommendations to adults with mental health and complex medical issues discharging from nursing facility settings utilizing a trauma informed harm reduction approach. Goal is to help clients to improve regain and develop the skills needed for day-to-day life. Treatment services are provided on-site at area nursing facilities and in client homes. Treatment is focused on community mobility and integration cognitive processes support with engagement in IADLs/ADLs hygiene/self-care routines sensory processing safety with transfers fall prevention and adaptive equipment to increase safety and independence. Must have Masters Degree and hold Illinois license in Occupational Therapy. Primary ResponsibilitiesConduct comprehensive functional assessments (ADLs IADLs cognition psychosocial skills) pre- and post transition.Develop and implement individualized treatment plans that aligns client-centered goals and values with transitioning into lower level of care.Implement evidence-based OT interventions (individual and/or group) utilizing an occupation based strengths based approachCollaborate with interdisciplinary teams (e.g. care/case managers SNF/SMHRF facilities staff RN staff housing specialists) to ensure continuity of care.Provide in-services or consultation to other team members regarding functional assessment adaptive strategies and environmental modifications.Attend meetings to consult with teams regarding client progress behavior and response to treatment.Instructs clients as needed in the use of adaptive equipmentAddress environmental modifications and recommend/provide adaptive equipment as needed for community/independent livingConduct interviews with family members guardians and/or significant others as appropriate about the clients past and present functional levels capabilities and performances.Provide community-based and in-home occupational therapy to facilitate skill-building for independent living (i.e. cooking budgeting transportation use)Promote self-efficacy community participation and social connection through structured activities and coachingMaintain accurate and timely documentation such as completing assessments written and verbal recommendations treatment plans progress notes and discharge summaries in accordance with agency policy Medicaid/Medicare and regulatory standards.Plans and/or assists with client discharge and provides appropriate recommendations for selection/use of adaptative equipment compensatory strategies and support programs as needed/identifiedProvide caregiver and client education on strengths and needs that support client success in community.Review medical clinical charts and/or other pertinent documents for the Class Member made available by the nursing facility and documentation from Care Management team as appropriate.Receive input from Supervisor and provide feedback regarding administrative concerns clinical consultations and caseloadsInterpret and communicate agency and program policies and proceduresParticipate in orientations and training opportunities as offered by the program and required for credentials/by the stateProvide direct supervision to Level II Occupational Therapy Students as assigned
Work Environment - A majority of the employees time will be spent in the community or nursing facility settings driving to and from Nursing Facilities and providing care in residences or community areas
- While performing the duties of this job the employee will have access to office space but will have ability to work remotely
- In supporting clients to live independently employee may be exposed to varying home conditions
Travel - The employee is required to travel by automobile to and from KYC office locations on a weekly basis as well as frequent client visits in nursing homes client residencies and nursing home facilities in the Chicagoland area and occasional meetings/trainings in and around the Chicagoland area.
Position Requirements - Masters Degree
- Occupational Therapist with current Illinois license
- One-year occupational therapy experience in nursing home setting or community based preferred
- Preferred experience with clients that experience mental health and complex medical conditions
- Current and valid drivers license required.
- Daily access to a personal vehicle is required.
Schedule/Timekeeping - Full Time Exempt 37.5 hour work week
