Key Responsibilities:
- Work collaboratively with Physical Therapists to execute individualized treatment plans.
- Support and guide patients through exercises functional tasks and daily living activities.
- Interpret evaluations and liaise with physicians to optimize therapy approaches.
- Maintain precise documentation and comply with legal and professional standards.
- Conduct follow-up visits for home health patients to assess eligibility for continued services.
- Provide maintenance therapy and assist patients in reaching their rehabilitation objectives.
- Regular travel to patient residences is required necessitating a valid drivers license.
Qualifications:
Home Health experience preferred but not required
Valid California Physical Therapist Assistant license required
Current CPR certification required
Current California drivers license good driving record and reliable transportation
