Overview
We are seeking an experienced Director of Patient Care Services to lead clinical operations for our Cleveland-based home health agency. This role is responsible for ensuring exceptional patient outcomes maintaining compliance with regulatory standards and driving operational and financial performance. The ideal candidate is a strategic leader with a passion for delivering high-quality compassionate care in a home-based setting.
Key Responsibilities
Leadership & Strategy
- Direct and oversee all patient care services ensuring alignment with organizational goals.
- Develop and implement care programs to enhance quality expand services and improve patient satisfaction.
- Build and maintain relationships with community organizations referral sources and healthcare partners.
Operations Management
- Own the P&L for the home health service line monitoring budget performance and implementing corrective actions as needed.
- Oversee the care delivery process ensuring efficiency timeliness and seamless coordination.
- Track KPIs analyze data and leverage technology (EHR systems) to optimize performance.
Compliance & Quality
- Ensure adherence to Medicare Medicaid and accreditation standards.
- Conduct quality reviews and implement evidence-based practices to improve patient outcomes.
- Identify and mitigate risks to patients and staff.
Team Development
- Recruit train and mentor clinical staff fostering a culture of accountability and professional growth.
- Provide ongoing feedback recognize achievements and support career development.
- Promote collaboration across interdisciplinary teams.
Patient & Family Engagement
- Ensure compassionate patient-centered care tailored to individual needs.
- Educate and guide patients and families on care plans and health goals.
- Represent the organization at community events and outreach programs.
Qualifications
- Bachelors degree in Nursing (BSN) Doctor of Physical Therapy (DPT) Healthcare Administration or related field required; Masters degree or advanced certification preferred.
- Minimum 5 years of home health experience including at least 2 years in a leadership role managing clinical operations.
- In-depth knowledge of OASIS Medicare/Medicaid regulations and accreditation requirements.
- Proven ability to lead interdisciplinary teams and drive operational and clinical excellence.
- Strong organizational communication and problem-solving skills.
- Proficiency with EHR systems (e.g. HomeCare HomeBase).
Work Environment
- Office-based with frequent field visits to oversee patient care support staff and engage referral sources.
- Fast-paced patient-focused setting requiring adaptability and decisive leadership.
- Local travel required within the service area.
