Job Description

Job Summary:

This Clinical Pharmacist provides comprehensive pharmaceutical care to Pediatric Solid Organ Transplant inpatient and outpatient services through integrated patient assessment and monitoring drug distribution clinical services research and teaching.

Actively supports the hospitals and departments continuous quality improvement and customer service goals individually and as an effective team member.

Job Responsibilities:

  • Participates in medical rounds for inpatient and outpatient Solid Organ Transplant service. Counsels Transplant medical team on drug therapies; specifically drug selection dosing administration pharmacokinetics pharmacology and patient outcomes or toxicities.
  • Provides drug information to the Solid Organ Transplant medical team pharmacy personnel and patients or legal guardians.
  • Completes medication history with patient or legal guardian facilitating medication reconciliation as necessary.
  • Provides discharge medication counseling; facilitating discharge and medication reconciliation as necessary. Facilitates discharge planning and smooth transition to alternate site of care or the outpatient pharmacy. Anticipates discharge date and medications. Assesses insurance drug coverage well in advance of discharge and assist in preauthorization for medications.
  • Enters and verifies Solid Organ Transplant service medication and/or induction orders. Ensures that transplant medication orders are appropriate according to the patients clinical status allergy drug histories and concomitant drug therapies.
  • Communicates with pharmacy nursing and medical staff any discrepancies or clarifications identified in Solid Organ Transplant patients.
  • Provides appropriate sign-out of Solid Organ Transplant patients to subsequent shifts and/or clinicians covering the Solid Organ Transplant service. Needs to be available for consultation on Solid Organ Transplant emergency issues.
  • Coordinates the preparation and/or review of Solid Organ Transplant protocols and order sets; advising the Transplant team as necessary.
  • Prevents identifies and documents adverse drug events (ADRs and ADEs) in the Safety Event Reporting System (SERS); providing follow-up to the appropriate areas or service.
  • Develops and maintains a system to facilitate the continuity of care or monitoring of Solid Organ Transplant patients as both inpatients and outpatients. Such a system should include patient specific profiles clinical notes and their immunosuppression or investigational drug records.
  • Follows patients listed but not yet transplanted to optimize pharmacologic therapy with regards to co-morbid conditions.
  • Assists in cardiopulmonary resuscitations and other medical emergencies.
  • Participates actively in committees or team meetings as determined by needs of the Transplant Center and pharmacy department.
  • Participates in research Medication Utilization Evaluations and Quality Assurance Projects as necessary for the pharmacy department.
  • Participates in the educational experience of healthcare professionals at Lurie Childrens; including medical and pharmacy staff and students/trainees.
  • Functions as a Clinical Pharmacist on a limited basis as determined by the Clinical Manager of Pharmacy Services.

Education and Experience:

  • Licensed or eligible for licensure as a pharmacist by the state of Illinois.
  • Completion of an ASHP-accredited Pharmacy Practice or Pediatric Specialty Residency with experience in pediatrics and/or transplantation or equivalent pediatric experience.
  • Develops good working relations with other hospital employees and creates a favorable image of Lurie Childrens in the eyes of patients visitors and the general public.

Pay Range

$124800.00-$205920.00 Salary
