Description

JES Tech is looking for a Legacy Health Case Manager (LHCM). This position is responsible for facilitating and coordinating clinical case management services in order to prevent unnecessary disability related to injury illness and hazardous exposures at the Hanford Site. The LHCM works as a key member of the Beryllium and Legacy Health Program team with oversight and direction from the Site Occupational Medical Director. The LHCM interfaces with Department of Energy (DOE) and Hanford contractor management state and federal workers compensation representatives attorneys internal and community-based medical preventive and behavioral health care providers and other stakeholders.


Responsibilities:

Program Support

  • Collaborates with clinic leadership to develop and implement methods to achieve established goals and objectives and ensure Best in Class status.
  • Develops and periodically reviews processes policies and procedures to ensure current evidence-based occupational health best practices and standard of care.
  • Maintains records for Legacy Health Program and related case management activities as appropriate.
  • Prepares reports including information related to case management related medical service activities correspondence and communication.
  • Works closely with and provides reciprocal backup support to the Beryllium Case Manager.

Operations / Communications

  • Serves as the clinical point of contact for the Legacy Health Case Management Program
  • Interfaces with stakeholders to maintain multi-directional communication that supports efficient high quality case management.
  • Provides counseling and education on the Legacy Health Program according to 10 CFR 851.
  • Coordinate referrals for screening evaluations and work capacity/suitability evaluations and other consultations related to case management issues.
  • Communicates with patients and serves as point of contact for follow-up after a new injury or exposure event including test results work restrictions and return to work.
  • Helps patients understand and navigate fitness for duty assessments work capacity/suitability evaluations and return to work services.
  • Works with patients providers and employers to clarify medical restriction recommendations and identify potential accommodations as needed.
  • Consults with medical preventive and behavioral health staff to coordinate a treatment plan that supports timely and safe return to work.
  • Coordinates clinical services and care plans with patients providers and employers.
  • Other related duties as assigned

Patient Care

  • Establishes a compassionate culturally sensitive patient-centered care environment.
  • Educates patients and promotes health wellness and lifestyle choices to improve well-being.
  • Participates as a member of the Exposure Response Team (along with the SOMD and other members of the Legacy Health Management Team) facilitating communication between patients providers and employer representatives related to emergency or exposure response.
  • Identifies signs and symptoms of injury illness and hazardous exposure and provides care according to nursing protocols policies and procedures. Occasionally this may include first aid and emergency response as a member of the clinic care team.
  • Provides safe high quality nursing care in accordance with internal policies and procedures local state (i.e. Washington Standards of Nursing Conduct or Practice; WAC) and federal regulations contract requirements AAAHC standards and standard of care.
  • Protects patients co-workers and self by adhering to standard precautions for infection prevention OSHA DOE and site-specific safety and health policies and procedures and AAAHC accreditation standards.
Requirements

Education/Experience

  • Bachelor of Science in Nursing (BSN) is required.
  • Valid unrestricted Washington State registered nurse (RN) license is required.
  • Minimum of three years experience in occupational health nursing with documented case management experience.
  • BLS and ACLS certification are required.
  • Certified Occupational Health Nurse (COHN) is desired.
  • Certified Case Manager (CCM) is preferred.


Skills/Training

  • Excellent verbal and written communication skills.
  • Ability to be flexible organized and maintain composure in stressful situations.
  • Ability to work independently and as a member of a multidisciplinary team.
  • Ability to communicate effectively and coordinate information from multiple stakeholders often from diverse backgrounds and perspectives.
  • Proficient in Microsoft Office (including Word Excel Outlook Teams).
  • Experience with electronic health records (Cority preferred).
  • Current knowledge of local community resources and medical trends is desirable.
  • Knowledge of current regulations and guidelines related to essential functions including familiarity with the Civil Rights Act of 1964 the Occupational Safety and Health Act of 1970 the Federal Rehabilitation Act of 1973 the Americans with Disabilities Act of 1990 and state and federal workers compensation rules.
  • Knowledge of laws and regulations governing protected health information (e.g. Privacy Act of 1974; Health Insurance Portability and Accountability Act of 1996) including demonstrated abilities related to maintaining privacy and confidentiality in communication correspondence and recordkeeping.
  • Advanced understanding of requirements related to release of protected health information to authorized parties.
  • Strong working knowledge of medical terminology and common patient care procedures.
  • Ability to maintain professional and technical knowledge by attending continuing professional education offerings reviewing professional publications and participating in professional organizations.

TYPICAL PHYSICAL DEMANDS & WORK ENVIRONMENT

Working Conditions

Work will mainly be performed in the general medical offices of the Hanford Occupational Medical Services Clinic. Shifts and locations may rotate between the clinics at 1979 Snyder Richland WA and 200 West on the Hanford site or any other location identified by leadership. Duties will involve frequent interactions with other employees customers clients and vendors. Work may be fast-paced and stressful at times. Occasionally work may be needed outside the typical schedule to accommodate client and/or operational needs. Opportunities for local travel may occur. The noise level in the work environment is usually minimal.


Physical Abilities

Able to walk sit and stand for extended periods of time; reach with hands and arms; balance; bend; stoop; talk and hear; have sufficient visual ability and manual dexterity to operate a keyboard computer calculator telephone and other such office equipment as necessary; occasionally move and/or lift up to 25 pounds or more with assistance.


Mental Abilities

Clear and conceptual thinking ability; excellent judgment troubleshooting problem solving analysis and discretion; ability to maintain composure while managing work-related stressors; ability to handle multiple priorities simultaneously; and ability to meet deadlines reliably. Must be mentally adaptable and flexible when interacting with a variety of people. Must be able to answer questions and provide guidance in a professional friendly and compassionate manner.

Comply with environment health safety and security (EHSS) policies and site-specific procedures. Participate in and support the Hanford EHSS program. Ensure safe working conditions within area of responsibility. Identify potential EHSS hazards concerns and incidents and report them to supervisory personnel.

Adhere to and foster acceptable health and safety practices in accordance with program requirements internal policies and procedures and applicable rules and regulations. Adhere to all company policies and procedures.

Be a U.S. Citizen or National and be able to successfully obtain a DOE security clearance.
