Description

We are currently seeking talented compassionate LPNs to provide rapid response hospice care to patients where they reside in Anne Arundel and Prince Georges Counties in Maryland.

This position is Full-time with a schedule of 3 12-hour shifts per week (8pm - 8:30am) plus the expectation of rotating Weekend coverage (Saturday & Sunday) based on patient-need and minimal holiday requirement.

This position is Non-Exempt and eligible for Shift Differential.

Want to join our Team We are currently looking for compassionate and dedicated LPNs to join our Rapid Response Team. Our Rapid Response LPNs under the direction of the registered nurse provide care to our patients in all settings including skilled and crisis nursing visits admissions death visits continuous care and GIP. Our Rapid Response LPNs may also provide Triage Support managing in-coming calls to answer questions locate the appropriate person or resource provide comfort and manage symptoms until a Nurse can be dispatched and arrive at the patients care setting.

Main Responsibilities:

  • Engage with patients to build trust while you assess their physical psychosocial and emotional condition as well as their environment during regular nursing visits. Adapts communication style to each individual and circumstance to provide an atmosphere that protects patient dignity.
  • Recognizes changes in clinical status anticipates patient and caregiver needs with a sense of urgency and reports any changes to the patient or their environment to the interdisciplinary team to ensure the highest level of service and care. Requests assistance from supervisor or an RN as needed.
  • Work collaboratively with your supervising RN Case Manager to initiate and revise plan and goals of care to ensure patient comfort and family/caregiver support and competence throughout their journey through advanced illness and end-of-life.
  • Follows nursing standards of practice and meets or exceeds HOC quality goals to ensure the best possible outcomes for the patient and family from admission through bereavement care.
