Where youll be working:

Based at 621 Hunter Street Newcastle the HNEKids Transition Care Service is part of Youth Health Services and provides vital coordination support to young people aged 1425 with chronic illness or disabilities. The service assists in the transition from paediatric to adult health care systems improving long-term engagement and outcomes.

About the role:

As Transition Care Coordinator youll support young people through a critical stage in their healthcare journey. Your role includes coordinating individualised care plans working directly with young people families and healthcare providers. Youll attend paediatric and adult health clinics support the development of self-management skills and follow up post-transition to ensure continued engagement in care.

This is a dynamic person-centred role with a focus on respect collaboration and empowerment. Youll be supported by the Senior Transition Care Coordinator and the Manager of Youth Health Services Children Young People and Families.


About you:

You are empathetic patient and able to build trusting relationships with young people and their families. Your strong communication and organisational skills enable you to effectively navigate complex care needs while managing competing priorities.

You thrive in a team environment are proactive in your learning and are committed to delivering coordinated high-quality care. Familiarity with healthcare systems is desirable but not essentialtraining and mentorship will be provided.


Requirements:

  • Relevant qualifications in Nursing Social Work Occupational Therapy Speech Pathology Physiotherapy or Dietetics providing eligibility for membership of the relevant professional association or full AHPRA registration
  • At least 3 years of clinical experience in the profession (Allied Health) or in the clinical area of the specified post-graduate qualification (for Clinical Nurse Specialist Grade 2)
  • Current NSW Drivers Licence and willingness to travel within the Local Health District
