Job Description

This pivotal role involves enhancing the quality of patient care in a home health setting ensuring that every patient receives compassionate personalized attention.
Key Responsibilities:
- Oversee and manage the delivery of patient care services in compliance with regulatory standards.
- Provide leadership guidance and support to nursing staff promoting professional development and improved patient outcomes.
- Collaborate with interdisciplinary teams to create and implement tailored patient care plans.
- Ensure timely and accurate documentation of patient care activities and progress.
- Perform regular audits of patient records and care plans to maintain quality assurance.
- Act as a liaison among patients families and healthcare providers to ensure effective care coordination.
- Participate in ongoing quality improvement projects and engage in professional development opportunities.
Required Qualifications:
- Minimum of two years of management or supervisory experience in a home care setting preferred.
- Knowledge of professional standards Medicare Conditions of Participation and compliance with federal state and local regulations.
Required Certifications and Licensures:
- Registered nurse with current licensure to practice nursing in the state of practice.
