Part Time Fixed Term Contract - 28/12/2025

Northeast Health Wangaratta (NHW) is the major health provider in the Central Hume region with an extensive range of acute sub-acute aged care and community services.

NHW is looking for a Community Rehabilitation Nurse to provide nursing care and leadership in the Subacute and Ambulatory Care Service (SACS) team . This will include but is not limited to assessment management and support of clients in the areas of: Cardiac Rehabilitation Pulmonary Rehabilitation Movement Disorders and general rehabilitation.

The successful applicant will work closely with the Manager of SACS and will require personal attributes that fosters collaboration between staff departments at NHW and relevant external applicant will need to be highly motivated and demonstrate knowledge and experience of working within community programs.
