Position Summary - Full-time position for Mental Health Case Manager to assist adult clients living with severe mental illness in Cincinnati/Hamilton County. Position includes working with clients who participate in specialized programs such as LGBTQ and older adult populations.
- Provide community-based case management services such as but not limited to symptom monitoring treatment/service planning assistance in daily living supportive services crisis intervention linkage/referral/advocacy to community support systems and support of clients self-determination.
- Work in a supportive team environment as well as collaboration with a multi-disciplinary clinical treatment team.
- Meet service delivery expectations.
What We Value in our next ADULT CASE MANAGER - High School Diploma or Equivalent bachelors degree is Preferred.
- Case Management experience is not required.
- Transportation and good driving record required as well as personal vehicle insurance for bodily injury/property damage liability.
- Strong interpersonal skills and ability to provide support in person and by telehealth.
- Must be able to lift/push/pull up to 20 pounds to support demands within office environment.
