This role is responsible for providing music therapy services to patients.
Location: High Pointe House Haverhill MA
Hours: Full-Time 40 Hours
Job Description
Minimum Qualifications:
1. Bachelors degree in Music Therapy.
Preferred Qualifications:
1. Board certification in Music Therapy (MT-BC)
Duties and Responsibilities: The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.
1. Assess music therapy needs of the patient/family and provide music therapy services to the hospice patients and their families as appropriate.
2. Consult with interdisciplinary team about patient referrals to assess needs and prioritize intervention.
3. Document all care provided on the patients clinical record in accordance with Hospices documentation requirements.
4. Educate hospice staff and community about the role of the Music Therapist and the use of music therapy interventions with hospice patients and their families.
5. Assist with facilitating support groups and special events upon request and ensure effective use of resources
6. Serve as an education resource to staff volunteers care partners and the community.
7. Support and promote the Hospice mission to lead in providing exceptional patient-centered end-of-life care and grief support.
8. Adhere to the practice of confidentiality regarding patients families staff and the organization.
9. Demonstrates a knowledge and understanding of what to report to the CEO or Chief Compliance Officer when concerns of corporate compliance arise.
10. Ensures compliance within guidelines set forth by regulatory agencies (DPH ERISA etc.) and demonstrates compliance with Home Health Foundation policies and procedures.
11. Practices confidentiality principles set by the agency and federal HIPAA/HITECH guidelines.
12. Participates in all mandatory in-services.
13. Completes other duties as assigned.
Full-Time
