Job Description

Working Title: Case Manager
Program: CSS - HAS
Reports To: Program Supervisor
Employment Type: Full Time Benefited 1.0 FTE (37.5 hours/week)
Hours: Monday
- Friday 8:30 AM to 5:00 PM
Location: Onsite Hood River Sherman and Wasco Counties in Oregon.Location will be based on the clients needs
Pay Range (Dependent on experience):
Level 1: $27.28-33.16/ hour

FLSA Status: Not-Exempt

This Case Manager supports the Housing Access Supports (HAS) program in the Community Support Services (CSS) department. The role provides case management services to assigned clients who need help seeking and securing housing. This position assesses eligibility assists clients in accessing housing and related services and collaborates with agency staff and community partners in connecting clients to resources. The Case Manager for HAS will also work with clients to improve functions and address basic needs to help the client reach their goal of housing.

The Case Manager provides case management services to assigned clients. This position assesses eligibility assists clients in accessing services and collaborates with Agency professionals in case planning and service coordination.

Mid-Columbia Center for Living (MCCFL) is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are both trauma-informed and recovery-oriented to persons / consumers with mental illness and addictions. Trauma-informed care includes safety choice collaboration trustworthiness cultural responsivity and empowerment for both clients and staff. MCCFL serves Hood River Wasco and Sherman Counties and utilizes a consumer-involved consumer-engaged and consumer-driven model. MCCFL provides a whole-person care approach to adults and children to address both physical and mental health needs. MCCFL offers a variety of programs including a mobile crisis unit intensive case management ongoing counseling community support supported employment jail diversion hospital discharges hearings and more.

What you need:
Any combination of education licensure and experience that demonstrates the ability to perform the positions essential responsibilities. This includes:
  • Certified as a Qualified Mental Health Associate (QMHA) by the Mental Health and Addiction Certification Board of Oregon (MHACBO) or register as QMHA-R within 14 days of hire.
  • Two (02) years of responsible experience in human services or related field is preferred.
  • Experience providing housing support services preferred.
  • Experience working with the severe and persistent mentally ill population is preferred.
  • Must successfully complete NSPIRE inspections training within 30 days of hire.
  • Bilingual fluency in Spanish/English is preferred.
  • Must possess a valid Oregon or Washington Drivers license and be insurable for client service purposes and for travel between business offices meetings and/or trainings.
  • Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR to .
What youll do:
  • Provide Case Management services to all assigned clients. Services are provided in a variety of community settings with consideration to client preference.
  • Assess the eligibility of potential clients utilizing agency standards and processes. Assist enrolled clients in acquiring services and resources needed to achieve treatment goals and maintain independence within the community. Coordinate with other community partners to develop provide and maintain a rich array of services and resources for clients.
  • Participate in medication appointments medication delivery and accurate and timely documentation in client record.
  • Actively participate in joint case planning and coordination with MCCFL multi-disciplinary teams that may be engaged with the client as well as agency psychiatrists.
  • Meet minimum client service productivity requirements as may be set by the Agency to meet requirements of outside funding and/or reimbursement sources.
  • Utilize agency clinical software system to prepare current client records and reports as necessary and required. Maintain timely and coherent documentation related to client needs and services adhering to Agency documentation policy. Ensure that all documentation is up-to-date and justifies the case management services provided.
  • Adhere to all clinical and MCCFL program policies and procedures. Remain updated on all applicable rules and Oregon Administrative Rules (OARs).
  • Participate in all team and agency meetings as scheduled. Attend local regional and/or state meetings as requested by Supervisor/Manager.
  • Within the scope of the position perform duties independently in the community and promote a positive image of the agency.
  • Maintain understanding of local behavioral health resources and Mid-Columbia Center for Livings role in that system.
  • Depending upon department this classification may provide after-hours emergency services on a 24-hour rotating basis such as telephone consultation crisis intervention and pre-hospital screening examinations if qualifications for this service are met. Such duties may be required as an essential responsibility based upon budget and/or staffing levels in specific departments and/or locations.
  • May provide some administrative support to the team. This may include tasks such as opening and closing case files greeting visitors and clients screening telephone calls etc.
  • Use agency vehicles to travel within the community provide transportation for clients and drive between business offices in order to complete essential responsibilities and attend required meetings and/or trainings.
  • Regular and reliable attendance; professional communication with all contacts; maintain confidentiality.
Additional Essential Responsibilities for the Case Manager II:
  • Conduct behavioral health screenings and/or mental health assessments for clients.
  • Determine and document client diagnoses.
  • Assess the clients family social and work relationships and provide supportive services where appropriate.
  • Create and oversee an individual plan of care.

Position-Specific Essential Responsibilities of Case Manager I or II in the Jail Diversion Program (This position may include any of the aforementioned general responsibilities plus the following):
  • Provide case management supports (including symptom monitoring skills training needs assessments) to Jail Diversion clients.
  • Provide and collaborate management services in a small team setting. Actively participate in joint case planning and coordination with agency multi-disciplinary team and agency clinicians.
  • Provide Community Restoration duties including Legal Skills Training/case management to .370 order Aid & Assist defendants to promote adjudication.
  • Participate in Mental Health Court proceedings; join community partners to case plan and support justice system involved clients.
  • Actively participate in meetings to collaborate with community partners including jail community corrections and housing systems; services are provided in a variety of community settings with consideration to client preference.
  • Must be able to meet the unique physical demands required of this role including but not limited to a high level of physical mobility and exertion and regular exposure to adverse environmental conditions.
