The Behavioral Health Coordinator/Bachelor of Social Work (BSW) provides social work assistance to patients and families dealing with mental health and substance use disorders life stressors and crises and stress-related physical symptoms. This role assists with the prevention diagnosis and treatment of these conditions. Working under close supervision this role advocates for patients by helping them access resources and coordinating care after discharge.
Responsibilities
Identifies and accesses resources to optimize effective and efficient patient behavioral care outcomes in collaboration with other members of the multidisciplinary team. Provides care coordination following discharge for patients and their families with various systems including hospitals schools department of social services and other community agencies. Assists the patient and families with personal and environmental difficulties which predispose the patient towards illness or interferes with obtaining maximum benefits from medical care. Maintains knowledge of and relationships with community health and social agencies to expand the behavioral health services available to patients via collaboration or referral. Assists the patient and family to understand medical recommendations and provide services planned to restore the patient to optimum social and health adjustments. Assists with paperwork and referral sources for patients being discharged from the hospital. Performs other duties as assigned.
Qualifications
EDUCATION:
Required: Bachelors in Social Work or related field
EXPERIENCE:
LICENSURE/CERTIFICATION/REGISTRY/LISTING:
Required: BLS (CPR)-American Red Cross or AHA Healthcare Provider
