Job Description
Overview
Responsibilities
Provides care coordination following discharge for patients and their families with various systems including hospitals schools department of social services and other community agencies.
Assists the patient and families with personal and environmental difficulties which predispose the patient towards illness or interferes with obtaining maximum benefits from medical care.
Maintains knowledge of and relationships with community health and social agencies to expand the behavioral health services available to patients via collaboration or referral.
Assists the patient and family to understand medical recommendations and provide services planned to restore the patient to optimum social and health adjustments.
Assists with paperwork and referral sources for patients being discharged from the hospital.
Performs other duties as assigned.
