How youll contribute
The Case Manager of Rehabilitation plays a vital role in coordinating patient access to care by responding to inquiry calls facilitating assessments and ensuring appropriate referrals within the community or facility. This role involves conducting or overseeing comprehensive assessments to identify medical behavioral and psychosocial needs including screening for emergency conditions. The Case Manager collaborates closely with the clinical team and on-call physicians to ensure timely and effective treatment planning for individuals with substance use disorders dual diagnoses or other psychiatric conditions. Additionally the Case Manager provides therapeutic support educates patients and families to promote understanding and engagement in care and manages administrative tasks such as admissions and pre-certification with external payers.

What were looking for
- Masters degree in social work or counseling required.
- RN licensure may be used in place of a masters degree
- Prior experience in a psychiatric healthcare facility working with adult and geriatric
- patients preferred.
- Must have strong clinical assessment skills.
- CPR and Handle with Care certified within 30 days.
- May be required to work flexible hours and overtime and ability to meet typing keyboard expectations. With proper credentials Specialist may be required to complete vitals check on patients.
