Job Description Summary:

Job Description:

Reports to physical therapist when re-evaluations are needed and or changes in patient status.
Delegates tasks and duties as indicated.
Performs documentation per policy and procedures ( progress notes discharge summary and multi-disciplinary education and treatment plan forms).

Additional Job Description:

Required:

PTA Licensure in practicing State
Associates as Physical Therapy Assistant

Preferred:

Physical Therapy Assistant or Rehab experience
