Case Manager Assistant Responsibilities

The Case Manager Assistant will assist the Case Managers in ensuring people gain access to needed and required healthcare. You will accompany people on scheduled appointments as directed by the Case Manager including but not limited to annual appointments and medical follow-up issues that may arise. The Case Manager Assistant will also take on urgent requests from the Case Manager or others to ensure that people receive medical treatment in a timely manner. The Case Manager Assistant will work with Operations Behavioral Health and Workforce Development staff to ensure accompaniment to all medical appointments. The Case Manager Assistant actively participates facilitates and contributes to the organizations ongoing mission to build and strengthen our internal culture with compassion forward thinking quality value and respect for diversity equity inclusion and belonging to enhance the lives of those we serve.

  • Responsible for accompanying people on scheduled medical appointments to include routine annual visits follow-up visits lab work sick appointments etc.
  • Responsible for communicating necessary medical information to practitioners to ensure proper medical attention for people.
  • Responsible for ensuring documentation of the appointment is completed prior to leaving the appointment.
  • Schedule all medical appointments for people as required and enter these in Outlook.
  • Maintain an up-to-date database related to scheduled appointments and efforts made to follow-up.
  • Create encounters in Practical Health Systems to correlate with scheduled appointments in Outlook.
  • Communicate scheduled appointments to the appropriate staff. Coordinate with Home Managers Direct Support Professionals Behavioral Health Staff and Workforce Development staff in facilitating timely attention to all persons medical needs.
  • Communicate with all staff to ensure proper preparation for specialty medical exams and procedures.
  • Ensure transportation for all people includes special needs of transportation or the need for additional staff to attend an appointment.
  • Responsible for correspondence with practitioner offices and medical facilities to request and acquire needed medical documentation.
  • Hand delivers properly filled out medical paperwork scripts etc. to the Case Managers to ensure appropriate and timely follow-through on physicians orders.
  • May perform the duties of the Direct Support Professional as necessary in accordance with current staffing plans.
  • Ensures confidentiality regarding sensitive material including applicants employees and individuals right to privacy and protected health information following Health Insurance Portability and Accountability Act (HIPAA).
  • Track time as required utilizing our internal online system.
  • Other duties as assigned by the Professional Services Director Professional Services Assistant Director Vice President of Professional Services and President.



Requirements
  • High school diploma or equivalent.
  • Department of Disabilities and Aging (DDA) designated online training First Aid CPR and Medication Certification course within the first 90 days of employment and any required training.
  • Work a flexible schedule as determined by supervisor based on the needs of people.
  • Ability to establish a comfortable and supportive relationship with individuals receiving support and their families.
  • Ability to maintain effective relationships with other management staff employees and the public demonstrating sensitivity to corporate needs employee goodwill and the Companys public image. Understanding and commitment to person-centered support for people with intellectual and developmental disabilities.
  • Ability to present facts and recommendations effectively in oral electronic and written form.
  • Ability to exercise good judgment responsibility and independent action.
  • Excellent crisis management interpersonal communication time management and organizational skills.
  • Uphold the strictest of confidentiality and follow the guidelines related to the Health Insurance Portability and Accountability Act (HIPAA)/HITECH and Family Educational Rights and Privacy Act (FERPA).
  • Utilize Electronic Health Record (EHR) as required for Company operations.
  • Intermediate computer skills.
  • Ability to lift 50 pounds (weight of average manual wheelchair when empty).
