How youll contribute
ASocial Worker who excels in this role:
Conduct comprehensive assessments to develop individualized culturally sensitive treatment plans for perinatal patients and NICU families. Deliver brief behavioral interventions and emotional support to pregnant and postpartum individuals as well as families with pediatric or NICU patients.
Provide crisis prevention and intervention services as needed.
Collaborate closely with the multidisciplinary healthcare team to ensure coordinated holistic care.
Offer bereavement support to families coping with loss.
Educate and inform the healthcare team about psychosocial cultural and trauma-related factors affecting patient and family engagement.
Promote cultural competence and equity in service delivery.
Advocate for patients healthcare rights and psychosocial needs.
Connect patients and families with appropriate community resources and support services.
Partner with Child & Family Services and other stakeholders to coordinate care effectively.
Provide follow-up support to patients and families post-discharge to ensure continuity of care.
Why Choose Us:
What were looking for
Masters in social work. Graduate of MSW program accredited by the Council of Social Work Education.
Preferred/Desire: Two years of experience in a clinic and/or medical setting.
Licensed social worker or candidate.
Certification and/or specific experience in perinatal behavioral health beneficial.
