Description

The Division of Endocrinology Diabetes and Bone Disease is providing care for patients with endocrine diseasesincluding but not exclusive to diabetes throughout the city by unifying protocols programs and treatments in the newly created Mount Sinai Health System to serve our community with endocrine disorders through outreach education and innovative community based programs.

Under the direction of the programs leadership the outpatient Diabetes Care Coordinator (DCC) acts as an integral part of the health care team at Mount Sinai physician network practice sites and/or faculty practice. The DCC formulates cohesive relationships with physicians administrators health care professionals and office staff at their assigned practice sites to foster the implementation and ongoing delivery of Diabetes Self-Management Education/Training (DSME/T) Medical Nutrition Therapy and diabetes patient care. The DCC evaluates patients diabetes skills knowledge of diabetes and patient care needs including their nutritional needs as well as other co-morbid conditions and risk for developing a diabetes related collaboration with the medical team creates customized care plans for the provision of appropriate nutrition and diabetes care as per the American Association of Diabetes Educators (AADE) guidelines.

The DCC is responsible for activities that support patient focused care and the achievement of the American Diabetes Association and American Heart Association patient care goals. These activities include but are not limited to responding promptly to patient referrals from physicians and health professionals performing diabetes obesity and cardiovascular patient assessments as per departmental protocols evaluating patients readiness to change goal setting evaluation of response to therapies providing DSMT/ MNT communicating variances in clinical indices to physicians and other members of the medical team and following through in treatment plan enhancement. The DCC serves to promote clinical excellence and cohesive relationships at assigned physician practices. The DCC adheres to the policies and requests of the Mount Sinai Doctors physician practices and/or faculty practice and supports the Medical Director Administrative and Clinical leadership of the practice in accomplishing their objectives.



Responsibilities
  • Program Implementation: Successfully facilitates the implementation of the Diabetes Alliance or other program related to obesity and cardiovascular disease at select Mount Sinai physician network practice sites. Actively engages network and faculty practice physicians their office manager administrative staff and health care professionals by: (1) explaining the objectives of the program (2) reviewing the implementation requirements including referral criteria and processes (3) articulating the program interventions value and communication processes (5) discussing program guidelines and means of documentation (7) highlighting diabetes self management educational topics and (8) reviewing outcome measures and end points.
  • Establishing Cohesive Clinical Relationships with Network Physicians: Establishes and builds cohesiveness clinical relationships with network physicians and Mount Sinai specialists at network physician practice sites by discussing patient specific cases opportunities to optimize patient care patients response to treatment plan enhancement and supports the dissemination of best practices surrounding diabetes self management education at each physician practice site.
  • Drives Clinical Value and Optimal Credibility: Performs patient care assessments diabetes self management education and builds customized care plans as per departmental guidelines with the medical team to reduce the risk of diabetes related complications optimize patient and provider satisfaction and patient care outcomes. Collaborates with physicians to update patient care plan and to support patients in understanding their treatment plan. Relates continuous glucose monitoring date as well as cloud connected glucometer data to the managing physician and follows-up with the patient in reinforce the updated treatment plan.
  • Focuses on Continual Quality Improvement: On an ongoing basis identities opportunities to enhance effectiveness of the Diabetes Alliance or other program. Identifies variances in patient care processes and nutritional related issues at the practice level. Documents strategies to positively influence patient care and works with the leadership to build and implement new standard operating procedures to enhance diabetes care.
  • Documents Clinical Effectiveness: Documents all patient encounters (face-to-face video visits and telephone) as per departmental protocols. Ensures that clinical process measures related to diabetes and cardiovascular disease are clearly documented. Supports patient goal setting and documents patients progress in achieving their goals as per protocols. Adheres to the American Diabetes Association guidelines for documenting patient goals patient education and outcomes related to patients knowledge base therapeutic lifestyle changes and self care activities.
  • Builds Nutritional and Weight Management Plans and Educational Offerings: Develops Mount Sinai branded culturally sensitive meal planning and weight loss collaterals that can be utilized across all patient care setting (inpatient ambulatory care and network physician practice sites) in support of preventing diabetes and/or weight loss for those with diabetes and or cardiovascular disease. Builds cohesive relationships with physicians from Diabetes Alliance Bariatric Surgery Diabetes Center and Cardiology to support the multidisciplinary approach to weight loss and healthy eating for those with diabetes at-risk for developing diabetes and cardiovascular disease which can be adopted at select physician network practice sites.
  • Supports the Development of a Medical Team for Patients: Establishes a medical team for patients with diabetes pre-diabetes and cardiovascular disease to support the goals of care outlined by the American Diabetes Association. Works effectively with Mount Sinai specialists (endocrinologists cardiologists bariatric surgeons and vascular surgery) and primary care physicians while working onsite at network physician practice sites to ensure patients have access to specialty care to avoid adverse events.
  • Community Outreach and Marketing: Represents Mount Sinai at community based Diabetes and Cardiovascular Events. Educates the public and at risk communities in a culturally sensitive manner on optimal nutritional practice to support overall wellness. Links patients with specialty services available at Mount Sinai to ensure access to diabetes and cardiovascular specialty care.
  • Supports the clinical integration of new diagnostic tools electronic medical records and health care technology into daily practice operations.
  • Supports the implementation of patient satisfaction and retention initiatives to support the ongoing growth of patients under management by the network.
  • Monitors practice to ensure compliance with: Mount Sinais compliance standards OSHA requirements HIPAA compliance emergency preparedness universal precautions JCAHO and practice certification standards and managed care provider standards.
  • Other duties and responsibilities as assigned.


Qualifications
  • Bachelors degree or higher from an accredited institution with completion of ACEND (Accreditation Council for Education in Nutrition & Dietetics)-accredited dietetic internship. Masters degree preferred.
  • On-going portfolio documentation of continuing education in diabetes care and nutrition in progress.
  • Minimum 5-10 years progressive experience in nutrition and dietetics including diabetes education. Demonstrated working within community-based diabetes hospital or physician practice on Diabetes Self-Management Education.
  • Registered dietitian (RD) from the Commission on Dietetic Registration (CDR) required
  • Certified Dietitian Nutritionist (CDN) from the NYS Department of Education Office of the Professions required or process initiated upon hire.
  • Advanced Certification Certified Diabetes Care and Education Specialist (CDCES).
