Description

The Clinical Program Manager for Geriatrics and Palliative Medicine supports the Vice Chair of Quality and Clinical Information and oversees the execution of departmental initiatives. This role ensures that all program goals and objectives are achieved according to defined priorities quality standards timelines and specifications. The program may operate as an independent initiative or as a component within the department or larger health system.

The Clinical Program Manager leads quality and performance improvement programs in collaboration with the departments quality leadership. They are responsible for managing documentation data reporting and coordination through Mount Sinai Hospital and Mount Sinai Health Systems quality infrastructure. This role combines project management education and cross-disciplinary engagement including direct interaction with departments administrators medical and quality leaders and clinical staff. The manager also oversees daily operations of core measures and clinical quality metrics and initiatives.



Responsibilities

Responsible To
Reports directly to the Vice Chair Quality and Clinical Innovation Brookdale Department of Geriatrics and Palliative Medicine and indirectly to the Associate Director of Quality and Informatics as applicable.

Responsible For

  • Ensuring accurate collection validation and reporting of quality and safety metrics including hospital core measures departmental metrics nursing-sensitive indicators and patient satisfaction data
  • Supporting an optimal practice environment for Relationship-Centered Care and quality improvement initiatives
  • Serving as a clinical liaison to the quality and clinical data analytics team

Principal Duties and Responsibilities

General

  • Manage multiple concurrent projects with detailed plans and task lists to ensure timely follow-through.
  • Collect and disseminate accurate quality and safety data across the Geriatrics and Palliative Care services support data interpretation for care improvement.
  • Oversee EHR templates to ensure usability and real-time data capture for clinical and leadership needs.
  • Prepare reports visualizations presentations and other media for quality safety and performance improvement communication.
  • Represent the department on internal and external committees and teams.
  • Maintain meeting minutes for quality committees and report outcomes to leadership.
  • Coordinate clinical data activities among clinical leads consultants and teams; ensure goal alignment and infrastructure.
  • Develop revise and implement program strategies to support sustainable growth.
  • Conduct needs assessments to inform program development.
  • Prepare orientation competency and evaluation reports tied to measurable outcomes.
  • Develop and manage work plans aligned with goals and resources; coordinate daily operations and prioritize activities.
  • Deliver timely status reports identify patterns and trends and recommend corrective actions.
  • Collaborate on the development and review of policies procedures and care standards.
  • Ensure compliance with hospital and regulatory agency standards; maintain readiness for accreditation surveys.
  • Perform additional duties as assigned by supervisory leadership.

Quality

  • Ensure accuracy and validity of quality data including manual chart reviews.
  • Collaborate with external programs and vendors as needed.
  • Promote alignment of quality initiatives across sites disciplines and care levels
  • Create and deliver data-driven presentations on quality outcomes and improvement goals.

Performance Improvement

  • Maintain an inventory of departmental performance improvement initiatives.
  • Coordinate interdisciplinary quality improvement efforts across the care continuum.

Professional Development

  • Participate in professional organizations and activities.
  • Engage in scholarly activities including public speaking and publishing.
  • Support a culture of professional practice and scholarship.
  • Demonstrate respect and inclusivity toward all patients families and colleagues.
  • Foster a culturally sensitive and responsive work environment.

Experience

  • Demonstrated success in quality and performance improvement and project management.
  • Experience in Geriatrics and Palliative Care preferred.
  • Proficiency with EPIC for clinical documentation and understanding of data structures (front-end and back-end) required.
  • Excellent verbal and written communication skills.
  • Strong ability to work independently and as part of interdisciplinary teams.
  • Capable of managing multiple projects concurrently.

Technical Skills

  • Intermediate to advanced Microsoft Office (Word Outlook Excel PowerPoint)
  • Proficiency in Tableau software for data visualization and reporting
  • Familiarity with database management tools (e.g. Redcap Access)
  • Experience with or willingness to learn statistical software (e.g. SAS SPSS Stata MATLAB R)
  • Physical capability to meet job responsibilities.


Qualifications
  • Baccalaureate Degree in Nursing is required. Masters in Nursing or advance provided is preferred. Certification in specialty area preferred.
  • Certification: Geriatrics and/or Palliative Care certification preferred
  • 5 years directly related experience (some supervisory preferred)
    New York State License with current registration as a registered professional nurse. Certification in specialty area preferred.
