As a Peer Support Specialist you will be a member of the Mobile Team assisting the clinicians on Mobile visits and providing support and advocacy to individuals in crisis. You will also be a resource to the clinicians and case manager through sharing their knowledge of common challenges individuals with SPMI endure and sharing knowledge about community resources. You will be an individual (adult) with lived experience with mental illness. You will report to the Clinical Site Supervisor and you will work on-site.
Responsibilities:
Collaborate with service providers involved with individuals with SPMI.
Provide information and peer to peer support to individuals who are served by the program.
Educate and empower clients and their supports regarding their roles in developing crisis plans and in crisis prevention.
Educate service providers regarding an individuals role in developing a crisis plan for themselves and in crisis prevention. Ensure that the individual is an integral part of the crisis planning process.
Inform service providers (including Mobile Crisis and PCFC providers) regarding person- centered and strength-based practices. Maintain and model a focus on strengths.
Utilize clinicians and clinical supervisor for advocacy for individuals served.
Ensure that clients are satisfied with the services they are receiving and if not help them advocate for what they need.
Educate individuals and primary caretakers on how to advocate if services are not meeting their needs.
Be the liaison to other local supports in the community.
Link individuals to Peer Support Services activities such as support groups social events newsletter.
Requirements and Education:
Requires a High School Diploma or GED
