What Youll Do in This Role:
We are seeking a compassionate and skilled nurse to join our PACE team! This role is responsible for providing both direct and indirect nursing care to PACE participants. While working under the direct and indirect supervision of the Clinic Supervisor or designee this nurse operates independently applying advanced nursing knowledge and clinical judgment. The ideal candidate is confident managing complex clinical situations and committed to delivering high-quality person-centered care to older adults.
Primary Responsibilities:
Assessment:
- Triages and responds to clinic phone calls.
- Collects participant data completes required documentation per PACE standards.
- Performs fall assessments in collaboration with the IDT (Interdisciplinary Team).
- Prioritizes participant needs and identifies referrals or follow-up as needed.
- Participates in staff meetings and care planning under Clinic Supervisor guidance.
- Conducts in-home assessments assigned (emergent needs only).
- Provides PTO coverage for Clinic RN.
Planning Care:
- Develops and maintains individualized care plans based on nursing process and interdisciplinary collaboration.
- Involves participant and family in care planning and revisions.
- Makes appropriate referrals to support services.
- Communicates changes in participant status to IDT members.
Implementing/Managing Care:
- Delivers care in accordance with clinical standards and protocols.
- Administer treatments including IV therapy wound care and tube feedings.
- Educates participants and families regarding 24-hour care coordination.
- Identifies and manages emergency situations independently.
- Demonstrates respect for participant rights and ethical considerations.
- Maintains accurate and timely documentation of care and clinical observations.
- Supports vaccine clinic scheduling and coordination.
- Serves as back-up for hospital case manager regarding admissions and discharges.
Leadership:
- Follows clinic workflows and protocols as outlined by leadership.
- Attends clinic meetings and contributes to quality improvement efforts.
- Delegates tasks appropriately using the nursing process.
- Integrates cost-effective strategies into care delivery.
- Addresses and resolve issues in care management including proper reporting.
- Participates in on-call rotation.
What Youll Need:
Required:
- Graduate of an accredited School of Nursing.
- Licensed Registered Nurse in the State of Michigan.
- CPR certified or willing to obtain upon hire.
Desired Qualifications:
- Three (3) years of experience as a practicing nurse in a health-related field.
- At least one (1) year of experience working with the frail or elderly population.
- Must have reliable transportation and possess a valid and current drivers license.
- Position requires the ability to lift and move objects weighting up to 50 pounds without assistance.
