JOB DESCRIPTION 0 EVALUATION FORM 0 POSITION TITLE: CATH LAB RN DEPARTMENT: CATH LAB REPORTS TO: DIRECTOR SUPERVISION GIVEN: NONE EMPLOYEE NAME: EVALUATION PERIOD: EVALUATION DUE: The following statements are intended to describe the major elements and requirements of the position and should not be taken as an all inclusive list of responsibilities duties and skills required of individuals assigned to this job. JOB SUMMARY: Provides direct patient care to adult and geriatric patient populations. Addresses the psychosocial physical and general aspects of care related to the surgical environment. Communicates with physician continuously and as needed about patient condition. Assists with the maintenance of equipment and inventory. Assesses procedure room or equipment functioning and readiness. Obtains supplies for individual cases and ensures all appropriate needs of the Cardiovascular Laboratory team are met. Monitors physiologic and hemodynamic data as related to diagnostic and/or therapeutic procedures performed in the cardiovascular lab. Monitors and records patients condition continuously while in the Cardiovascular lab. Participates in performance improvement and continuous quality improvement activities. AGE-SPECIFIC COMPETENCIES Employees working in this capacity must meet age-specific competencies related to the following categories: Neonate/Infant: Birth 12 months Child / Pediatric: 1 12 years Adolescent: 13-17 years Adult: 18 64 years Geriatric: 65 and over EDUCATION EXPERIENCE TRAINING 1. Bachelor Degree in Nursing (BSN); preferred. 2. Active state Registered Nursing (RN) License; required. 3. Current BCLS (AHA) ACLS certificate upon hire and maintain current; required. 4. One (1) year clinical experience in a Cardiovascular Lab doing cardiac work or in a Critical Care area; required. PERFORMANCE EVALUATION INSTRUCTIONS: SCORE EACH CATEGORY BASED ON THE FOLLOWING CRITERIA: 3 MEETS AND EXCEEDS ALL STANDARDS 2 MEETS ALL STANDARDS 1 MEETS SOME STANDARDS -SOME IMPROVEMENT NEEDED 0 IMPROVEMENT NEEDED ***NOTE: Please refer to Position Responsibilities and Evaluation Ratings for more detailed information about the ratings. METHOD: (O) OBSERVATION (D) DEMONSTRATION (V) VERBALIZED SECTION I: PERFORMANCE ACCOUNTABILITY A. KNOWLEDGE OF WORK RATING 1. Completes focused assessment on patients upon admission to the procedure room including history allergies pain assessment and code status. 2. Initiates updates and/or ensures accurate entry of data in the procedural record. 3. Explains all procedures to patients maintaining privacy and allowing patients to express emotional needs. Knowledge of IV medications ADR and blood components 4. Documents evidence of nursing care on the procedure record. Observes for reactions and/or complications and report to the physician in a timely manner. Participates in the PI data collection regarding core measures and pain management. 5. Knowledgeable about National Patient Safety Goals. B. DUTIES AND RESPONSIBILITIES RATING 1. Consistently uses two patient identifiers when drawing lab administering medications and performing procedures. 2. Does not use unacceptable abbreviations in documentation and using read back on all telephone orders. Monitors authentication and validation of telephone orders within 24 hours by the physician. 3. Consistently follows medication reconciliation policy. 4. Ensures Crash Cart defibrillation function have been checked per protocol and after each code appropriate items are immediately replaced and indicated by signature on crash cart checklist. 5. Is in compliance with information contained in the Infection Manual (i.e. OSHA bloodborne pathogen standards CDC Guidelines) and demonstrates appropriate use of personal protective equipment ( i.e. gloves gowns masks goggles ) and hand washing te
