Job Description

Position Title: Alternative Education Student Case Manager

Pay Grade: Per Instructional Salary Scale Period of Employment: 10 months

Description Approved/ Revised: July 28 2025 FLSA Status: Exempt

Supervised by and/or Reports to: Coordinator of Alternative Education Programs

General Responsibilities:

The case manager for students enrolled in alternative education programs or placements serves a vital role in supporting students who encounter challenges during their educational journey. This position involves collaborating with students families and various school and community stakeholders to develop and implement and monitor individualized support plans that address academic social and emotional needs for students in alternative education settings. The primary goal of the Student Case Manager is to ensure students receive the necessary resources and assistance to thrive in a learning environment ultimately helping them achieve their educational and personal goals with a focused emphasis on meeting the requirements for high school graduation. The case manager will be involved in each step of the transition process for students removed from the traditional school setting to an alternative setting and will also work with school teams to facilitate the transition back to a traditional setting.

Essential Duties

  • Serve as the primary point of contact for students navigating enrollment processes including class registration course selection guidance and resolution of enrollment-related inquiries.
  • Ensure the maintenance of accurate and comprehensive student records encompassing academic transcripts attendance logs and other pertinent information.
  • Foster regular communication with students families and service providers to enhance collaboration and maintain clear effective dialogue.
  • Collect analyze and report on data concerning student success attendance and other critical metrics to inform decision-making.
  • Continuously monitor student progress in academic and behavioral domains implementing strategies to enhance engagement and attendance.
  • Collaborate with school staffteachers counselors and administratorsto cultivate a supportive and inclusive learning environment for all students.
  • Identify and address challenges faced by students whether they pertain to academic performance behavioral issues or social-emotional well-being.
  • Advocate for students needs ensuring they access the necessary support and resources to thrive academically and personally.
  • Organize and lead monthly multidisciplinary team meetings (MDT) with all relevant stakeholders including home-based instructors attendance officers counselors and support staff to promote student success.
  • Involved in each step of the transition process with students moving from the traditional school setting to an alternative setting and also collaborates with school teams for the transition back to a traditional setting.
  • Other duties as assigned to support students and staff achieve optimal outcomes for students within the alternative education programs.

Requisite Knowledge Skills and Abilities:

  • Ability to balance multiple priorities and meet established deadlines.
  • Strong ability to work independently take initiative and problem-solve creatively.
  • Excellent oral and written communication organizational presentation and public relations skills.
  • Understanding of diverse cultural ethnic and socio-economic backgrounds.
  • Strong interpersonal skills with diverse student populations.
  • Knowledge of school policies and student information systems.
  • Good time management and organizational skills.
  • Ability to work independently and as part of a team.
  • Experience with data collection and analysis.
  • Proven professionalism and composure in stressful situations.
  • Bilingual in Spanish preferred.
  • Experience in case management including follow-ups on appointments and referrals.
  • Effective at managing multiple tasks and projects efficiently.
  • Consistently models professionalism in all settings.

Requisite Education and Experience:

  • A Bachelors Degree is required preferably in counseling education social work criminal justice psychology or a related field.
  • A minimum of three years of successful experience working with youth intervention programs or directly with youth who are at-risk of not graduating from high school due to academic behavioral and/or emotional challenges.

Physical Requirements:

Must have the use of sensory skills in order to effectively communicate and interact with other employees and the public through the use of the telephone and personal contact as normally defined by the ability to see read talk hear handle or feel objects and controls. Physical capability to effectively use and operate various items of office related equipment such as but not limited to: a personal computer calculator copier and fax machine. Some climbing stooping kneeling crouching reaching standing walking pushing pulling lifting grasping or repetitive motions required.
