The Opportunity
Reporting to the Manager of Inpatient and Ambulatory Medical Care the Licensed Practical Nurse (LPN) is a member of an interprofessional healthcare team who collaborate together in the delivery of care to meet the needs of patients and their families within the Pediatric Consultation Clinic portfolio. The LPN practices independently in collaboration with and under the direction of the Registered Nurses and/or physicians and/or other health care providers in the provision of care through assessment treatment and education to those patients and their families with medical illnesses/conditions.
Responsibilities include:
- Providing holistic patient and family-centered care to children and their families.
- Monitor and assist in patient flow during clinics.
- Assessing patient and family needs during clinic visits; communicating pertinent patient/family information to the appropriate interprofessional team member(s).
- Provides telephone facilitated care and documents appropriately according to the Health Centre policies.
- Collaborating with the care team members (e.g. RNs physicians etc.) to develop and implement an individualized plan of care.
- Consulting other health care providers as needed in the delivery of family-centered care.
- Collaborating with the Clinical Leader of Shared Clinics and other staff as appropriate in the implementation of quality initiatives including the development and maintenance of clinical and operational policies protocols standards guidelines and educational material.
- Acting as a clinical resource to other health centre staff regarding patient care issues specific to pediatric medical conditions.
- Participating in team-specific program and other health center committees as appropriate including Pediatric Consultation Clinic general meetings MOM Committee and Accreditation-related committees.
- In collaboration with the team members and the Clinical Leader of Shared Clinics participating in identifying issues that have potential or actual impact on clinical care.
- Participating in the orientation of learners and new staff to the clinic and Shared Clinics as appropriate.
- Collaborating with Registered Nurses in Shared Clinics as needed.

Your Qualifications
- Graduate from an approved Practical Nurse Diploma Program required; 2-year diploma program preferred.
- Active license with the Nova Scotia College of Nursing (NSCN) required.
- Successful completion of pharmacology and health assessment courses is required.
- Sound knowledge of the role and scope of practice of the LPN within an ambulatory setting is preferred.
- Successful completion of CPR Level-C Healthcare Provider within the last 12 months is required.
- Recent clinical experience that includes the administration of medications and health assessments required.
- Recent clinical experience in pediatric care and/or ambulatory care preferred.
- Demonstrated understanding and champion of family-centered care is required.
- Demonstrated assessment and child/family teaching skills required.
- Demonstrated ability to work as a member of an interprofessional health care team as well as identify the need for and accept direction as required.
- Demonstrated critical thinking within the scope of practice including clinical judgment problem-solving skills and strong decision-making ability required.
- Demonstrated professional behavior and strong interpersonal and communication (verbal & written) skills required.
- Demonstrated initiative excellent time management and organizational skills are required.
- Advanced working knowledge of Windows MS Office programs Outlook Meditech and Internet searches preferred.
- Competency in other languages is considered an asset.
